Description
Overview of the medicine
Dermastin 0.05 mg Ointment contains fluticasone propionate, a potent corticosteroid used to treat inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses such as eczema and psoriasis.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin areas once or twice daily, as directed by a physician. Do not use for more than 4 weeks without reassessment.
Elderly
Same as adult dosage, but caution with prolonged use due to potential for skin thinning and increased systemic absorption.
Renal_impairment
No specific dosage adjustment required, as systemic absorption is minimal.
How to Take
For external use only. Apply a thin layer to the affected skin area. Wash hands before and after application. Avoid contact with eyes and mucous membranes. Do not use with occlusive dressings unless advised by a doctor.
Mechanism of Action
Fluticasone propionate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory, antipruritic, and vasoconstrictive properties. It inhibits phospholipase A2, leading to decreased synthesis of prostaglandins and leukotrienes, which are mediators of inflammation.
Pharmacokinetics
Onset
Relief of symptoms typically within a few days of application.
Excretion
Excreted mainly in the feces, with a small amount in urine.
Half life
Not well defined for topical application due to minimal systemic absorption, but systemically absorbed fluticasone propionate has a terminal half-life of approximately 7.8 hours.
Absorption
Minimal systemic absorption through intact skin; absorption increases with skin damage, occlusion, or application to large surface areas.
Metabolism
Primarily metabolized in the liver via CYP3A4 to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to fluticasone propionate or any component of the ointment
- Primary cutaneous infections (bacterial, fungal, viral, parasitic)
- Perioral dermatitis
- Acne vulgaris
- Rosacea
- Perianal and genital pruritus
- Children under 1 year of age
Drug Interactions
CYP3A4 inhibitors (e.g., Ritonavir, Ketoconazole)
May increase systemic exposure to fluticasone propionate, leading to potential systemic corticosteroid effects. Clinical significance is minimal for topical use, but caution advised with concomitant use of strong inhibitors.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children. Protect from light and moisture.
Overdose
Acute overdose is unlikely with topical use. Chronic excessive use or misuse may lead to systemic corticosteroid effects like Cushing's syndrome, hyperglycemia, or adrenal suppression. Treatment is symptomatic and supportive; gradual withdrawal of the corticosteroid may be necessary.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit outweighs the potential risk to the fetus. Lactation: Use with caution, as it is unknown if fluticasone propionate is excreted in human milk. Avoid applying to the breast area.

Extensive clinical trials have established the efficacy and safety of topical fluticasone propionate for corticosteroid-responsive dermatoses.
Lab Monitoring
- Not routinely required for topical use, but if systemic absorption is suspected with extensive or prolonged use, adrenal function tests (e.g., plasma cortisol) may be considered.
Doctor Notes
- Counsel patients on proper application technique and duration of therapy.
- Monitor for signs of skin atrophy, infection, or systemic effects.
- Avoid abrupt discontinuation after prolonged use on large areas due to risk of rebound flares.
Patient Guidelines
- Use exactly as directed by your doctor.
- Do not cover the treated area with bandages or other dressings unless directed by the doctor.
- Avoid prolonged use, especially on the face, groin, or axillae.
- Do not use for diaper rash.
