Description
Overview of the medicine
Dermomix Cream is a combination medicine used to treat various inflammatory skin conditions where there is also a bacterial infection, or a risk of bacterial infection. It contains a corticosteroid (Betamethasone Dipropionate) to reduce inflammation, itching, and redness, and an antibiotic (Fusidic Acid) to kill bacteria causing or aggravating the infection.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Do not use for more than two weeks without medical advice. Total treatment duration should not exceed 14 days.
Elderly
No specific dose adjustment is required. Use with caution for prolonged periods.
Renal_impairment
No specific dose adjustment is required for topical application.
How to Take
For topical use only. Apply a small amount to the affected area and gently rub it in. Wash hands thoroughly before and after application, unless hands are the treated area. Avoid contact with eyes, mouth, and other mucous membranes.
Mechanism of Action
Betamethasone Dipropionate is a potent corticosteroid that acts by inducing phospholipase A2 inhibitory proteins (lipocortins), thereby controlling the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes. Fusidic Acid is an antibiotic that inhibits bacterial protein synthesis, primarily by interfering with the translocation step of elongation factor G during protein synthesis. This leads to the inhibition of growth and multiplication of susceptible bacteria.

Onset
Symptom relief (itching, redness) typically begins within a few days of consistent application.
Excretion
Betamethasone metabolites are primarily excreted by the kidneys. Fusidic acid and its metabolites are mainly excreted in the bile and faeces.
Half life
Systemic half-life of Betamethasone is approximately 5.6 hours. Fusidic acid's systemic half-life is 5-6 hours.
Absorption
Minimal systemic absorption of both active ingredients occurs after topical application to intact skin. Absorption may be increased through damaged skin, prolonged use, or with occlusive dressings. Systemic absorption of Betamethasone can be up to 5-6%. Fusidic acid absorption is negligible.
Metabolism
Systemically absorbed Betamethasone is primarily metabolized in the liver to inactive metabolites. Fusidic acid is extensively metabolized in the liver, mainly by conjugation.

Contraindications

Overdose
Acute overdose is unlikely due to the nature of the topical application. Prolonged or excessive use may lead to local adverse effects such as skin atrophy, striae, or systemic effects of corticosteroids (e.g., Cushing's syndrome, adrenal suppression) if significant absorption occurs. Management involves gradual withdrawal of the drug.
Pregnancy & Lactation
Pregnancy: Use with caution during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid large areas, prolonged use, or occlusive dressings. Lactation: It is not known whether topically administered corticosteroids are absorbed systemically in sufficient amounts to produce detectable quantities in breast milk. Use with caution during lactation; do not apply to the breast area.
