Description
Overview of the medicine
Desloratadine is a non-drowsy antihistamine used to relieve allergy symptoms such as sneezing, runny nose, itchy or watery eyes, and skin rash (hives). It works by blocking a natural substance (histamine) that your body makes during an allergic reaction.
Uses & Indications
Dosage
Adults
5 mg once daily.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with renal or hepatic impairment.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), 5 mg every other day. Dosage adjustment may be required in moderate renal impairment.
How to Take
Desloratadine tablets may be taken with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Desloratadine is a tricyclic antihistamine with selective peripheral H1-receptor antagonist activity. It prevents histamine from binding to H1 receptors, thereby alleviating allergic symptoms. It also has anti-inflammatory effects.
Pharmacokinetics
Onset
Within 30 minutes to 1 hour.
Excretion
Primarily excreted in urine and feces as metabolites.
Half life
Approximately 27 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations (Cmax) are achieved approximately 3 hours after dosing.
Metabolism
Extensively metabolized to 3-hydroxydesloratadine, an active metabolite, primarily via CYP2C8 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to desloratadine, loratadine, or any component of the formulation.
- History of allergic reactions to antihistamines.
Drug Interactions
Ketoconazole, Erythromycin, Azithromycin, Fluoxetine, Cimetidine
May increase plasma concentrations of desloratadine and its active metabolite, but generally without clinically significant adverse effects.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Desloratadine is not removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Desloratadine is excreted in breast milk, so caution is advised in nursing mothers; consider discontinuing nursing or the drug.
