Description
Overview of the medicine
Desopil 30 is a combined oral contraceptive (COC) pill used to prevent pregnancy. It contains two female hormones, Desogestrel (a progestogen) and Ethinylestradiol (an estrogen). It works primarily by preventing ovulation.
Uses & Indications
Dosage
Adults
One tablet daily for 21 consecutive days, followed by a 7-day pill-free interval. A new pack is started after the 7-day interval.
Elderly
Not indicated for postmenopausal women.
Renal_impairment
No specific dose adjustment is generally needed, but caution should be exercised in severe renal impairment.
How to Take
Take one tablet orally at approximately the same time each day, regardless of meals. Follow the arrows on the blister pack to take the pills in the correct order.
Mechanism of Action
This medication works by inhibiting ovulation, altering the cervical mucus to hinder sperm penetration, and changing the endometrial lining to make it less receptive to implantation. The primary mechanism is the suppression of gonadotropins (LH and FSH), which prevents follicle development and ovulation.
Pharmacokinetics
Onset
Contraceptive protection starts after 7 consecutive days of proper use, or immediately if started on Day 1 of the menstrual cycle.
Excretion
Etonogestrel and its metabolites are excreted in urine and feces. Ethinylestradiol is excreted in urine and feces as glucuronide and sulfate conjugates.
Half life
Desogestrel (as etonogestrel) has a terminal elimination half-life of approximately 20-30 hours. Ethinylestradiol has a terminal elimination half-life of about 10-20 hours.
Absorption
Both Desogestrel and Ethinylestradiol are rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Desogestrel is rapidly metabolized in the intestinal wall and liver to its active metabolite, etonogestrel. Ethinylestradiol undergoes extensive first-pass metabolism, primarily through hydroxylation by cytochrome P450 enzymes.
Side Effects
Contraindications
- •Presence or history of venous or arterial thrombotic/thromboembolic events (e.g., DVT, PE, stroke, MI).
- •Known or suspected sex-hormone-influenced malignancies (e.g., breast cancer).
- •Undiagnosed vaginal bleeding.
- •Severe liver disease or liver tumors.
- •Migraine with aura.
- •Pregnancy.
Drug Interactions
Lamotrigine
COCs may decrease lamotrigine plasma concentrations, reducing its therapeutic effect.
Ritonavir, Nelfinavir
Can significantly reduce the plasma concentrations of estrogen and progestogen, potentially leading to contraceptive failure.
Certain antibiotics (e.g., Ampicillin, Tetracycline)
May decrease contraceptive efficacy by altering gut flora and enterohepatic recirculation (evidence is limited and controversial).
Enzyme inducers (e.g., Rifampicin, Phenytoin, Carbamazepine, Barbiturates, St. John's Wort)
May decrease contraceptive efficacy due to increased metabolism of hormones.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, and withdrawal bleeding in females. There is no specific antidote; treatment is symptomatic and supportive.
Pregnancy & Lactation
Desopil 30 is contraindicated during pregnancy. If pregnancy occurs while taking the pill, discontinue immediately. Not recommended during breastfeeding as it may reduce the quantity and quality of breast milk and small amounts of steroid hormones may pass into breast milk.
