Overview of the medicine
Dexamethasone is a potent synthetic glucocorticoid with anti-inflammatory and immunosuppressive properties. It is used to treat a wide range of conditions, including allergic disorders, skin conditions, ulcerative colitis, arthritis, lupus, psoriasis, and breathing disorders.
Uses & Indications
Dosage
Adults
Initial dose varies from 0.5 mg to 9 mg daily, depending on the condition. Maintenance dose adjusted based on patient response. For most inflammatory conditions, 0.5-2 mg daily. For severe conditions, higher doses may be used.
Elderly
Use lowest effective dose. Monitor for adverse effects.
Renal_impairment
No dosage adjustment usually required.
How to Take
Take orally with food or milk to minimize GI upset. Take exactly as prescribed. Do not stop abruptly.
Mechanism of Action
Dexamethasone binds to glucocorticoid receptors, modifying gene expression, leading to anti-inflammatory and immunosuppressive effects.

Side Effects
Contraindications

Drug Interactions
NSAIDs
Increased risk of GI ulceration.
Anticoagulants
May alter anticoagulant effect.
Insulin/Oral hypoglycemics
May increase blood glucose, requiring dose adjustments.
Potassium-depleting diuretics
Increased risk of hypokalemia.
Phenytoin, barbiturates, rifampicin
May decrease dexamethasone efficacy.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Acute overdose is rare; chronic overdose can lead to Cushingoid features. Treatment is supportive, and gradual withdrawal of the drug if chronic.
Pregnancy & Lactation
Pregnancy Category C: Use only if potential benefit justifies potential risk to the fetus. Lactation: Excreted in breast milk; caution advised, weighing benefits vs. risks.
