Description
Overview of the medicine
Dexogut 30 mg Capsule contains Dexlansoprazole, a proton pump inhibitor (PPI) used to decrease the amount of acid produced in the stomach. It is prescribed for the healing of erosive esophagitis (acid-related damage to the esophagus) and for the symptomatic treatment of non-erosive gastroesophageal reflux disease (GERD).
Uses & Indications
Dosage
Adults
Healing of EE: 60 mg once daily for up to 8 weeks. Maintenance of EE: 30 mg once daily. Symptomatic non-erosive GERD: 30 mg once daily for up to 4 weeks.
Elderly
No dosage adjustment necessary based on age alone. However, caution should be exercised in very elderly patients due to potential comorbidities.
Renal_impairment
No dosage adjustment is needed for patients with mild to moderate renal impairment. For severe renal impairment, use with caution due to limited data.
How to Take
Take Dexogut capsule orally, with or without food. Swallow the capsule whole; do not chew, crush, or open the capsule.
Mechanism of Action
Dexlansoprazole is a proton pump inhibitor that suppresses gastric acid secretion by irreversibly inhibiting the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells. This action specifically blocks the final step in acid production.

Acid suppression begins within a few hours of administration.
Excretion
Approximately 60% of the dose is excreted in urine and 23% in feces as inactive metabolites.
Half life
Approximately 1-2 hours (terminal elimination half-life).
Absorption
Absorbed after oral administration, exhibiting a dual delayed-release profile. The first release occurs within 1 hour, and the second release occurs 4-5 hours later. Bioavailability is about 76%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP) isoenzymes CYP2C19 and CYP3A4, to inactive metabolites.

- Hypersensitivity to dexlansoprazole or any other component of the formulation
- Known hypersensitivity to other proton pump inhibitors
Drug Interactions
Digoxin
Increased systemic exposure to digoxin has been reported; monitor digoxin levels.
Warfarin
Increased INR and prothrombin time have been reported with concomitant PPI use, requiring monitoring.
Clopidogrel
Potential for reduced antiplatelet effect of clopidogrel; consider alternative antiplatelet therapy.
Methotrexate
Possible increased and prolonged elevation of serum methotrexate levels and/or its metabolite, particularly with high doses.
Atazanavir, Nelfinavir
Dexlansoprazole may reduce the absorption of these antiretrovirals, leading to decreased efficacy. Avoid co-administration.
