Description
Overview of the medicine
Dezacot 6 mg Suspension contains Deflazacort, a corticosteroid used to treat various inflammatory and autoimmune conditions, as well as Duchenne muscular dystrophy. It works by suppressing the immune system and reducing inflammation.
Uses & Indications
Dosage
Adults
Dosage varies widely depending on the condition. For inflammatory conditions, 6-90 mg daily, adjusted as needed. For Duchenne Muscular Dystrophy, typically 0.9 mg/kg/day.
Elderly
May require dosage adjustment, starting with lower doses due to potential decreased renal or hepatic function.
Renal_impairment
Generally no specific dose adjustment required for renal impairment, but caution is advised.
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal irritation. Measure the dose accurately using a measuring spoon or cup. Shake the bottle well before use.
Mechanism of Action
Deflazacort is a glucocorticoid with anti-inflammatory and immunosuppressive properties. It binds to intracellular glucocorticoid receptors, forming a complex that translocates to the nucleus and modulates gene expression. This leads to the synthesis of anti-inflammatory proteins and inhibition of pro-inflammatory mediators, thereby reducing inflammation and immune response.
Pharmacokinetics
Onset
Varies by condition; typically within hours to days for anti-inflammatory effects.
Excretion
Primarily excreted via the kidneys (urine) as metabolites, with some fecal excretion.
Half life
Active metabolite has a plasma half-life of approximately 1.5-2 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and converted to its active metabolite, 21-desacetyl deflazacort.
Metabolism
Primarily metabolized in the liver to the active metabolite, 21-desacetyl deflazacort, which is then further metabolized.
Side Effects
Contraindications
- Hypersensitivity to deflazacort or any components
- Systemic fungal infections
- Administration of live or live-attenuated vaccines during treatment
Drug Interactions
Anticoagulants
May alter the effects of anticoagulants; monitoring of INR is recommended.
Ketoconazole, Erythromycin
May increase deflazacort levels by inhibiting its metabolism.
Diuretics (Thiazide or Loop)
Increased risk of hypokalemia.
Phenytoin, Phenobarbital, Rifampicin
May decrease deflazacort efficacy by increasing its metabolism.
Non-steroidal anti-inflammatory drugs (NSAIDs)
Increased risk of gastrointestinal ulceration.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include Cushingoid features, hyperglycemia, and electrolyte disturbances. Treatment is supportive and symptomatic; gradual withdrawal if chronic overdose.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Small amounts excreted in breast milk; caution advised. Consult a doctor.
