Description
Overview of the medicine
Diafre is a non-drowsy antihistamine used to relieve symptoms associated with seasonal allergies, such as sneezing, runny nose, itchy or watery eyes, and itchy throat or nose. It works by blocking histamine, a natural substance that the body produces during an allergic reaction.
Uses & Indications
Dosage
Adults
10 mg once daily.
Elderly
No dosage adjustment necessary, but use with caution in those with severe hepatic or renal impairment.
Renal_impairment
For creatinine clearance <30 mL/min, 10 mg every other day.
How to Take
Oral, can be taken with or without food.
Mechanism of Action
Loratadine is a long-acting, non-sedating antihistamine with selective peripheral H1-receptor antagonist activity. It prevents the effects of histamine by competitively binding to H1 receptors without significant central nervous system penetration, thus causing less drowsiness.
Pharmacokinetics
Onset
Within 1-3 hours.
Excretion
Approximately 40% excreted in urine and 42% in feces as conjugated metabolites over a 10-day period.
Half life
Parent drug: 8.4 hours (range 3-20 hours); Active metabolite (desloratadine): 28 hours (range 8.8-92 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations achieved in 1-1.5 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP3A4 and CYP2D6) to desloratadine (an active metabolite).
Side Effects
Contraindications
- •Hypersensitivity to Loratadine or any component of the formulation

Drug Interactions
CYP3A4 and CYP2D6 inhibitors
Concomitant use may increase loratadine levels.
Ketoconazole, Erythromycin, Cimetidine
May increase plasma concentrations of loratadine and desloratadine, potentially increasing side effects.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, tachycardia, and headache. Management is symptomatic and supportive, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, so caution should be exercised when administered to a nursing mother.
