Overview of the medicine
Dilator 20 mg tablet is a nitrate vasodilator used to prevent and treat angina pectoris (chest pain) caused by coronary artery disease. It works by relaxing and widening blood vessels, improving blood flow to the heart.
Uses & Indications
Dosage
Adults
Initially 20 mg once daily, increasing to 20 mg twice daily after 1 week if needed. Doses should be separated by 7 hours to avoid tolerance (e.g., 8 AM and 3 PM).
Elderly
Similar to adult dosage, but caution required due to increased sensitivity and potential for hypotension.
Renal_impairment
No specific dosage adjustment required, but use with caution.
How to Take
Take orally with a glass of water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Isosorbide mononitrate is denitrated to nitric oxide (NO) in vascular smooth muscle cells. NO activates guanylate cyclase, leading to an increase in cyclic GMP (cGMP). cGMP causes dephosphorylation of myosin light chains, resulting in relaxation of smooth muscle, predominantly in veins, leading to venodilation and reduced preload. It also causes arterial dilation, reducing afterload.
