Description
Overview of the medicine
Dilol 625 mg Tablet is a beta-blocker primarily used for the management of hypertension, angina pectoris, and certain types of cardiac arrhythmias. It works by blocking beta-adrenergic receptors, leading to a decrease in heart rate, blood pressure, and myocardial contractility.
Uses & Indications
Dosage
Adults
For hypertension: Initial dose 625 mg once daily, may be increased to 1250 mg once daily after 1-2 weeks based on response. For angina/heart failure: Doses should be carefully titrated upwards starting from lower strengths (if available).
Elderly
Lower initial dose may be considered, and titration should be slower due to potential for increased sensitivity and decreased renal function.
Renal_impairment
No dose adjustment generally needed for mild to moderate impairment. For severe renal impairment (CrCl < 30 mL/min), dose reduction by 50% or increased dosing interval may be required. Consult physician.
How to Take
Dilol 625 mg tablet should be taken orally, with or without food, preferably at the same time each day to maintain consistent blood levels. Do not crush or chew the tablet.
Mechanism of Action
Dilol selectively blocks beta-1 adrenergic receptors in the heart, leading to a reduction in heart rate and contractility, thereby decreasing cardiac output. It may also have some peripheral vasodilatory effects. By reducing the heart's workload, it helps lower blood pressure and relieve angina symptoms.
Pharmacokinetics
Onset
Antihypertensive effect usually observed within 1 hour, full effect within 1-2 weeks of regular dosing.
Excretion
Approximately 40% excreted unchanged in urine, and the remainder as metabolites via renal and fecal routes.
Half life
Elimination half-life typically ranges from 6 to 10 hours.
Absorption
Well absorbed orally with approximately 60-80% bioavailability. Peak plasma concentrations are reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2D6 and CYP3A4 enzymes, with several active and inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to Dilol or other beta-blockers
- •Second- or third-degree atrioventricular (AV) block (without a pacemaker)
- •Sick sinus syndrome (without a pacemaker)
- •Cardiogenic shock
- •Overt cardiac failure (unless compensated)
- •Severe bradycardia (heart rate < 45-50 bpm)
- •Severe uncontrolled asthma or chronic obstructive pulmonary disease (COPD)
Drug Interactions
Digitalis Glycosides
Increased risk of bradycardia and AV block.
Insulin and Oral Antidiabetics
Beta-blockers may mask symptoms of hypoglycemia (e.g., tachycardia) and alter glucose metabolism. Adjust antidiabetic dosage as needed.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Dilol.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia, AV block, and hypotension. Monitor cardiac function closely.
Catecholamine-depleting drugs (e.g., Reserpine, Guanethidine)
May have an additive effect, leading to excessive reduction in sympathetic tone (e.g., bradycardia, hypotension).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe bradycardia, hypotension, acute cardiac failure, and bronchospasm. Management involves supportive therapy. Atropine for bradycardia, intravenous fluids and vasopressors for hypotension, and glucagon for refractory cases. For bronchospasm, beta-2 agonists may be administered.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Dilol is excreted in breast milk. Exercise caution when administered to a nursing woman; consider discontinuing nursing or the drug, taking into account the importance of the drug to the mother.
