Overview of the medicine
Disogen 2 mg Tablet is a potassium-sparing diuretic used to treat conditions such as edema, hypertension, and heart failure by helping the body excrete excess salt and water while retaining potassium.
Uses & Indications
Dosage
Adults
Initial dose 2 mg daily. May be increased to 4 mg daily if needed, based on clinical response and serum potassium levels. Take with food to enhance absorption.
Elderly
Start with 2 mg daily. Monitor electrolyte levels closely. Dose adjustment may be needed due to altered renal function.
Renal_impairment
Contraindicated in severe renal impairment (creatinine clearance <30 mL/min). Use with extreme caution and reduced frequency in mild to moderate impairment, with strict electrolyte monitoring.
How to Take
Oral administration. Take the tablet with or without food, preferably at the same time each day. Taking it with food may improve absorption and reduce gastrointestinal upset.
Mechanism of Action
Disogen acts as an aldosterone antagonist, competing with aldosterone for receptor sites in the renal distal tubules and collecting ducts. This inhibits sodium reabsorption and potassium excretion, leading to increased water excretion and potassium retention.

Excreted primarily in urine (27-50%) and feces (35-40%), mainly as metabolites.

- Hyperkalemia (serum potassium >5.5 mEq/L).
- Severe renal impairment (creatinine clearance <30 mL/min) or anuria.
- Addison's disease.
- Concomitant use of other potassium-sparing diuretics or potassium supplements.

May reduce diuretic and antihypertensive effects; increased risk of hyperkalemia and renal impairment.
Lithium
Disogen may reduce renal clearance of lithium, leading to increased lithium levels and toxicity.
