Overview of the medicine
Valsartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure, heart failure, and to improve survival after a heart attack.
Uses & Indications
Dosage
Adults
For hypertension: Initial 80-160 mg once daily, adjusted up to 320 mg. For heart failure: Initial 40 mg twice daily, increased to 160 mg twice daily as tolerated.
Elderly
No specific dose adjustment required, but cautious titration.
Renal_impairment
No initial dose adjustment for mild to moderate renal impairment. Caution with severe impairment.
How to Take
Administer orally, with or without food.
Mechanism of Action
Blocks the binding of angiotensin II to AT1 receptors, preventing vasoconstriction and aldosterone secretion, leading to vasodilation and reduced blood pressure.

Onset
Onset of action within 2 hours, peak effect within 6 hours.
Excretion
Primarily excreted unchanged in feces (83%) and urine (13%).
Half life
Approximately 6-9 hours.
Absorption
Rapidly absorbed, bioavailability ~23%.
Metabolism
Minimally metabolized; 20% metabolized to an inactive metabolite.
Side Effects
Contraindications
- •Pregnancy (second and third trimesters)
- •Concomitant use with aliskiren in diabetic patients
- •Severe hepatic impairment
Drug Interactions
NSAIDs
Reduced antihypertensive effect, increased risk of renal impairment, especially in elderly or volume-depleted patients
Lithium
Increased serum lithium levels and toxicity
Potassium-sparing diuretics/potassium supplements
Increased risk of hyperkalemia
Storage
Store below 30°C in a dry place, away from moisture and direct sunlight. Keep out of reach of children.
Overdose
Symptoms: Hypotension, tachycardia, bradycardia. Management: Symptomatic and supportive treatment, IV fluids for hypotension. Valsartan is not dialyzable.
Pregnancy & Lactation
Category D in 2nd and 3rd trimesters due to fetal toxicity. Not recommended during breastfeeding due to potential risk to the infant.
