Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg to 400 mg daily. This can be given as a single daily dose or in two divided doses (e.g., 100 mg every 12 hours). For 100mg capsules, typically 1-2 capsules every 12 hours.
Elderly
No specific dose adjustment is required for elderly patients with normal renal function.
Renal_impairment
Dose adjustment is recommended for patients with creatinine clearance less than 60 mL/min. For CrCl 21-60 mL/min, 75% of the standard dose; for CrCl < 20 mL/min, 50% of the standard dose.
How to Take
Can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid onset of action.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. No significant biliary or fecal excretion.
Half life
3-4 hours.
Absorption
Approximately 40-50% orally absorbed, absorption is slightly delayed by food but not significantly affected.
Metabolism
Minimal metabolism occurs.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime, other cephalosporins, or penicillins.
- •Patients with a history of severe allergic reactions to beta-lactam antibiotics.
Drug Interactions
Carbamazepine
May increase plasma carbamazepine levels when co-administered.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time (INR), requiring close monitoring and dose adjustment of anticoagulants.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by dialysis. Supportive therapy should be instituted as necessary.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Caution should be exercised when administered to a nursing mother, as Cefixime may be excreted in breast milk.
