Description
Overview of the medicine
Dolwin is a combination medicine containing Paracetamol (a non-opioid analgesic) and Tramadol (an opioid analgesic). It is used to relieve moderate to severe pain.
Uses & Indications
Dosage
Adults
Usually 1-2 tablets every 4-6 hours as needed, not exceeding 8 tablets per day.
Elderly
Similar to adults, but adjust for renal/hepatic impairment. Maximum 6 tablets per day for patients over 75 years.
Renal_impairment
Dose adjustment required. Increase dosing interval to 12 hours for CrCl < 30 mL/min.
How to Take
Oral, with or without food. Swallow whole, do not crush or chew.
Mechanism of Action
Paracetamol acts centrally to inhibit prostaglandin synthesis. Tramadol acts as a weak opioid agonist and also inhibits norepinephrine and serotonin reuptake, modulating pain perception.

Mainly renal (kidney) excretion.
Half life
Paracetamol: 2-3 hours; Tramadol: 5-7 hours
Absorption
Both components are rapidly and extensively absorbed after oral administration. Bioavailability of paracetamol is high, tramadol is about 75%.
Metabolism
Primarily hepatic (liver) metabolism, extensively metabolized to active and inactive metabolites.
Side Effects
Contraindications

- •Acute intoxication with alcohol, hypnotics, opioid analgesics, psychotropic drugs
- •Severe respiratory depression
- •Uncontrolled epilepsy
- •Severe hepatic impairment
Drug Interactions
Warfarin
Increased INR, bleeding risk.
Carbamazepine
Significantly reduces tramadol plasma levels.
MAO Inhibitors
Increased risk of serotonin syndrome, seizures. Concomitant use is contraindicated.
SSRIs, SNRIs, TCAs
Increased risk of serotonin syndrome and seizures.
CNS depressants (alcohol, hypnotics, sedatives)
Increased risk of respiratory depression and profound sedation.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include respiratory depression, seizures, coma, hepatic failure (from paracetamol). Management involves supportive care, naloxone for tramadol, and N-acetylcysteine for paracetamol.
Pregnancy & Lactation
Not recommended during pregnancy (especially third trimester due to neonatal withdrawal). Use in lactation should be avoided as both components are excreted in breast milk.
