Don-A 5 mg Pediatric Drop contains Domperidone, a medication used to treat nausea and vomiting, and to relieve symptoms of digestive discomfort associated with delayed gastric emptying in children.
Uses & Indications
Dosage
Adults
Not indicated for adults in this pediatric drop formulation. For children (up to 12 years): 0.25-0.5 mg/kg body weight, 3-4 times daily before meals. Max daily dose: 1 mg/kg, not exceeding 30 mg/day.
Elderly
Not indicated for elderly in this pediatric drop formulation.
Renal_impairment
In severe renal impairment (CrCl < 30 mL/min), frequency should be reduced to once or twice daily, depending on severity.
How to Take
Administer orally, preferably 15-30 minutes before meals. Use the provided calibrated dropper for accurate measurement of the prescribed dose.
Mechanism of Action
Domperidone acts as a peripheral dopamine D2 receptor antagonist, primarily in the gastrointestinal tract and the chemoreceptor trigger zone (CTZ). By blocking these receptors, it promotes gastric emptying and intestinal peristalsis, thereby reducing nausea and vomiting.

About 66% excreted in feces and 33% in urine over 4 days, mainly as metabolites.

Metabolism
Extensively metabolized in the liver via N-dealkylation and oxidative hydroxylation, primarily by CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Domperidone or any excipients
- •Prolactin-releasing pituitary tumor (prolactinoma)
- •Moderate to severe hepatic impairment
- •Conditions where stimulation of gastrointestinal motility might be harmful (e.g., gastrointestinal hemorrhage, mechanical obstruction, perforation)
- •Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
- •Patients with known existing prolongation of cardiac QTc intervals, significant electrolyte disturbances, or underlying cardiac diseases
Drug Interactions
Anticholinergic drugs
May antagonize the prokinetic effect of Domperidone.
Dopamine agonists (e.g., Bromocriptine, Cabergoline)
Domperidone may antagonize their effects due to its dopamine receptor blocking activity.
QT-prolonging drugs (e.g., Amiodarone, Haloperidol, Cisapride)
Increased risk of cardiac arrhythmias; concomitant use generally not recommended.
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin, Clarithromycin)
Concomitant use is contraindicated due to increased plasma levels of Domperidone and heightened risk of QT prolongation and cardiac arrhythmias.
Storage
Store below 30°C, in a dry place, away from direct sunlight and moisture. Keep tightly closed and out of the reach of children. Do not freeze.
Overdose
Symptoms of overdose may include drowsiness, disorientation, extrapyramidal reactions (especially in children), and QT prolongation. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered if recent ingestion. ECG monitoring is recommended.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnant women only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Domperidone is excreted in breast milk in small amounts; caution should be exercised and a doctor consulted before use during lactation.
