Overview of the medicine
Doripenem is a carbapenem antibiotic used to treat various severe bacterial infections, including complicated intra-abdominal and urinary tract infections, and pneumonia.
Uses & Indications
Dosage
Adults
500 mg (2 vials of 250 mg) intravenously every 8 hours, infused over 1 hour. Treatment duration depends on the type and severity of infection, typically 5-14 days.
Elderly
No dose adjustment usually needed unless severe renal impairment (CrCl ≤ 50 mL/min).
Renal_impairment
Adjust based on CrCl: CrCl 30-50 mL/min: 250 mg IV every 8 hours; CrCl 10-30 mL/min: 250 mg IV every 12 hours. Not recommended for CrCl <10 mL/min.
How to Take
For intravenous infusion only. Reconstitute each 250 mg vial with 10 mL of sterile water for injection, then dilute in 50-100 mL of 0.9% sodium chloride injection or 5% dextrose injection. Administer over 1 hour.
Mechanism of Action
Doripenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell death.

Rapid onset due to intravenous administration.
Excretion
Primarily renal, with approximately 70% of the dose excreted unchanged in urine via glomerular filtration and tubular secretion.
Half life
Approximately 1 hour.
Absorption
Administered intravenously, resulting in 100% bioavailability.
Metabolism
Primarily metabolized to an inactive ring-opened metabolite by dehydropeptidase-I in the kidneys.
Side Effects
Contraindications
- •Known severe hypersensitivity to doripenem or other carbapenem antibiotics.
- •Known severe hypersensitivity (e.g., anaphylactic reaction) to other beta-lactam antibiotics (e.g., penicillins or cephalosporins).
Drug Interactions
Probenecid
Increases doripenem plasma concentrations and half-life by inhibiting renal tubular secretion; not recommended for co-administration.
Valproic Acid
Doripenem may significantly decrease valproic acid levels, leading to loss of seizure control. Co-administration is generally not recommended.
Storage
Store unconstituted vials at 20°C to 25°C (68°F to 77°F), with excursions permitted to 15°C to 30°C (59°F to 86°F). Protect from light. Once reconstituted or diluted, solutions should be used promptly.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Hemodialysis can remove doripenem from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Doripenem is excreted in breast milk; caution advised during lactation.
