Description
Overview of the medicine
Doripen 500 mg injection is a broad-spectrum carbapenem antibiotic used to treat severe bacterial infections, including complicated intra-abdominal infections, complicated urinary tract infections, and pneumonia.
Uses & Indications
Dosage
Adults
500 mg administered by intravenous infusion over 1 hour every 8 hours.
Elderly
No specific dosage adjustment is generally required for elderly patients with normal renal function; monitor renal function.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance ≤ 50 mL/min. Refer to product information for specific dosing.
How to Take
Administer by intravenous infusion over 1 hour. It must be reconstituted and diluted prior to use.
Mechanism of Action
Doripenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is bactericidal.
Pharmacokinetics
Onset
Rapid onset of action after intravenous administration.
Excretion
Primarily excreted unchanged by the kidneys (approximately 70% to 80% of the dose).
Half life
Approximately 1 hour.
Absorption
Administered intravenously, resulting in 100% bioavailability.
Metabolism
Primarily metabolized by dehydropeptidase-1 (DHP-1) in the kidney, although it is relatively stable to DHP-1.
Side Effects
Contraindications
- •Hypersensitivity to doripenem or other carbapenem antibiotics.
- •History of severe hypersensitivity reaction to any other beta-lactam antibacterial agents (e.g., penicillins, cephalosporins).
Drug Interactions
Valproic Acid
Co-administration with doripenem can decrease valproic acid plasma concentrations, potentially leading to subtherapeutic levels and increased risk of seizures.
Oral Anticoagulants
May potentiate the anticoagulant effect of warfarin and other oral anticoagulants.
Storage
Store unopened vials at controlled room temperature (20-25°C) and protect from light. Reconstituted and diluted solutions should be used within specified timeframes.
Overdose
In case of overdose, discontinue doripenem and institute general supportive treatment. Hemodialysis may be useful in enhancing the elimination of doripenem.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Doripenem is excreted in human milk; caution should be exercised when administered to a nursing woman.
