Overview of the medicine
Dotfix 1 mg Tablet is an antidepressant medicine primarily used to treat major depressive disorder, generalized anxiety disorder, and panic disorder. It works by increasing the levels of serotonin in the brain, a natural substance that helps maintain mental balance.
Uses & Indications
Dosage
Adults
Initial dose 1 mg once daily, may be increased to 5-10 mg daily after several weeks based on response. Max dose 20 mg/day.
Elderly
Initial dose 1 mg once daily, consider lower maintenance doses due to reduced metabolism.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food, preferably at the same time each day. Do not crush or chew the tablet.
Mechanism of Action
Dotfix works by selectively inhibiting the reuptake of serotonin in the presynaptic neuronal cleft, thereby increasing the concentration of serotonin available to bind to postsynaptic receptors. This leads to an enhancement of serotonergic neurotransmission in the central nervous system.

Onset
Initial therapeutic effects may be observed within 2-4 weeks, with full effect often seen after 6-8 weeks.
Excretion
Excreted mainly through urine (renal) and feces (biliary), with negligible unchanged drug.
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with peak plasma concentrations reached within 4-6 hours. Bioavailability is high.
Metabolism
Primarily hepatic metabolism via CYP2C19 and CYP3A4 into active and inactive metabolites.
Side Effects
Contraindications

Drug Interactions
NSAIDs
Increased risk of gastrointestinal bleeding.
Pimozide
Increased Pimozide plasma levels, QT prolongation.
Warfarin
Potentiation of anticoagulant effects, increased bleeding risk.
MAO Inhibitors
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, tremor, somnolence, nausea, vomiting, tachycardia, and seizures. Management is symptomatic and supportive; maintain an open airway, monitor cardiac and vital signs. Activated charcoal may be considered if ingested within 1-2 hours.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to SSRIs late in the third trimester have developed complications requiring prolonged hospitalization, respiratory support, and tube feeding. It is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
