Overview of the medicine
Dotfix 2 mg Tablet is an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder, and irritability associated with autistic disorder.
Uses & Indications
Dosage
Adults
Usually 1-2 mg once or twice daily, titrated based on response and tolerability. Max 16 mg/day.
Elderly
Lower starting doses (e.g., 0.5 mg BID) and slower titration; careful monitoring due to increased sensitivity and risk of side effects.
Renal_impairment
Initial dose 0.5 mg BID, then titrate to 1-2 mg/day, maximum 4-6 mg/day with caution.
How to Take
Take orally with or without food. Do not crush or chew prolonged-release tablets. Take consistently at the same time each day.
Mechanism of Action
Risperidone primarily acts as a potent antagonist of serotonin 5-HT2 and dopamine D2 receptors. It also antagonizes alpha1-adrenergic and H1-histaminergic receptors.

Excretion
Excreted renally (70%) and fecally (14%) as unchanged drug and metabolites.
Half life
Risperidone: ~3 hours; Active metabolite (9-hydroxyrisperidone): ~21 hours.

Well absorbed orally, bioavailability is about 70%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6, to 9-hydroxyrisperidone (Paliperidone), an active metabolite.
Side Effects
Contraindications
- •Hypersensitivity to risperidone or any component of the formulation.

Drug Interactions
Antihypertensive agents
Increased hypotensive effect.
Dopamine agonists (e.g., Levodopa)
Risperidone may antagonize effects of dopamine agonists.
CYP2D6 inhibitors (e.g., Fluoxetine, Paroxetine)
May increase plasma concentrations of risperidone and 9-hydroxyrisperidone.
CNS depressants (e.g., Alcohol, Opioids, Benzodiazepines)
Increased CNS depression and sedation.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Treatment is symptomatic and supportive; maintain airway, monitor cardiac function, and provide appropriate hydration.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Infants exposed to antipsychotic drugs during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Excreted in breast milk, so caution is advised; benefits to mother versus risks to infant should be considered.
