Description
Overview of the medicine
Doximar 200 mg Tablet is a bronchodilator used for the treatment of bronchial asthma and chronic obstructive pulmonary disease (COPD). It helps to open up the airways in the lungs, making breathing easier.
Uses & Indications
Dosage
Adults
Typically 200-400 mg twice daily, adjusted based on clinical response and tolerability.
Elderly
Similar to adults, but dosage may need to be reduced in elderly patients with co-morbidities or impaired liver function. Start with a lower dose.
Renal_impairment
No significant dosage adjustment is usually required in mild to moderate renal impairment, but caution is advised in severe renal impairment.
How to Take
Take the tablet orally with or without food. Swallow whole with water, do not crush or chew.
Mechanism of Action
Doxofylline is a xanthine derivative that acts as a bronchodilator. It exerts its therapeutic effect by inhibiting phosphodiesterase enzymes, primarily PDE4, leading to an increase in intracellular cyclic AMP (cAMP) levels. This results in the relaxation of bronchial smooth muscles and subsequent bronchodilation. It also exhibits anti-inflammatory properties.
Pharmacokinetics
Onset
Bronchodilatory effects generally begin within 30-60 minutes.
Excretion
Primarily excreted in the urine as metabolites.
Half life
Approximately 6-8 hours in adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily via oxidation, to inactive metabolites. Less than 4% is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to doxofylline or other xanthine derivatives (e.g., theophylline, aminophylline).
- Acute myocardial infarction.
- Severe hypotension.
- Severe hepatic impairment.
- Lactation.
Drug Interactions
Beta-adrenergic agonists
May have an additive bronchodilatory effect.
Phenobarbital, Phenytoin, Rifampicin
May decrease plasma concentrations of doxofylline, reducing its efficacy.
Fluoroquinolones (e.g., Ciprofloxacin)
Concurrent use with xanthines may increase the risk of xanthine toxicity.
Cimetidine, Erythromycin, Clarithromycin, Allopurinol
May increase plasma concentrations of doxofylline, potentially leading to increased side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, epigastric pain, restlessness, insomnia, headache, tachycardia, arrhythmias, and in severe cases, convulsions. Management involves symptomatic and supportive treatment, including gastric lavage, activated charcoal, and monitoring vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Doxofylline is excreted in breast milk, therefore contraindicated during lactation.

Clinical Trials
Doxofylline has undergone various clinical trials to establish its efficacy and safety in the management of asthma and COPD, demonstrating comparable bronchodilatory effects to theophylline but with a better tolerability profile.
Lab Monitoring
- No routine specific lab monitoring is required. However, in patients with hepatic or renal impairment, monitoring for adverse effects and dose adjustment may be necessary.
Doctor Notes
- Consider dose adjustment in patients with significant hepatic impairment.
- Monitor for signs of xanthine toxicity, especially if co-administered with drugs that inhibit its metabolism.
- Advise patients about potential CNS and GI side effects.
Patient Guidelines
- Take Doximar regularly as prescribed by your doctor.
- Do not exceed the recommended dose.
- Report any unusual or persistent side effects to your doctor.
- Do not stop taking the medicine suddenly without consulting your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
