Description
Overview of the medicine
Doxolator is an oral solution containing 100 mg of an active compound per 5 ml, primarily used for the treatment of various forms of depression and anxiety disorders. It belongs to the tricyclic antidepressant class, working by balancing certain neurotransmitters in the brain.
Uses & Indications
Dosage
Adults
Initial dose 25-50 mg orally at bedtime, gradually increasing to 100-300 mg daily in divided doses or as a single bedtime dose. Max 300 mg/day.
Elderly
Lower initial dose (e.g., 10-25 mg at bedtime) and slower titration due to increased sensitivity and potential for adverse effects. Max 150 mg/day.
Renal_impairment
No specific dose adjustment for mild to moderate impairment, but caution advised. For severe impairment, dose reduction may be necessary.
How to Take
Oral. Can be taken with or without food. To minimize sedation, larger portion of daily dose can be taken at bedtime.
Mechanism of Action
Doxolator primarily inhibits the reuptake of norepinephrine and serotonin into presynaptic neurons, thereby increasing the concentration of these neurotransmitters in the synaptic cleft. This leads to an improvement in mood and reduction in anxiety.
Pharmacokinetics
Onset
Antidepressant effects may take 2-4 weeks to manifest, while sedative effects can be immediate.
Excretion
Primarily excreted in urine as metabolites, with a small amount excreted in feces.
Half life
Variable, ranging from 10 to 25 hours for the parent compound and active metabolites.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver via cytochrome P450 enzymes, primarily CYP2D6 and CYP3A4, to active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to doxolator or any component
- Recent myocardial infarction
- Concomitant use with MAOIs
- Acute angle-closure glaucoma
- Severe hepatic impairment
Drug Interactions
MAOIs
Serious, potentially fatal reactions including hyperthermia, rigidity, myoclonus, autonomic instability, and mental status changes. Avoid concomitant use; a washout period is required.
Anticholinergics
Potentiated anticholinergic effects (dry mouth, blurred vision, constipation, urinary retention).
CNS Depressants (alcohol, sedatives, hypnotics)
Increased CNS depressant effects.
Cytochrome P450 Inhibitors (e.g., cimetidine, fluoxetine)
May increase plasma levels of doxolator.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children.
Overdose
Symptoms include severe drowsiness, confusion, agitation, hallucinations, blurred vision, dry mouth, urinary retention, dilated pupils, fever, fast or irregular heartbeat, seizures, and coma. Management involves symptomatic and supportive care, gastric lavage, activated charcoal, and monitoring vital signs. Physostigmine may be used for severe anticholinergic symptoms.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Excreted in breast milk, caution advised. Consult doctor.
