Description
Overview of the medicine
Doxonil 400 mg Tablet contains Doxofylline, a bronchodilator used to treat bronchial asthma, COPD, and associated conditions. It works by relaxing the smooth muscles of the airways.
Uses & Indications
Dosage
Adults
400 mg twice daily, or as directed by physician. May be increased to 600 mg twice daily in some cases.
Elderly
Lower doses may be required, e.g., 200 mg twice daily, adjusted based on clinical response and tolerability.
Renal_impairment
No dose adjustment generally required, but caution is advised in severe impairment.
How to Take
Oral administration. Tablets should be swallowed whole with water, preferably after meals to minimize gastrointestinal upset.
Mechanism of Action
Doxofylline is a xanthine derivative that acts as a phosphodiesterase inhibitor, leading to an increase in intracellular cAMP levels. It also has antagonistic activity on adenosine receptors, although its precise mechanism differs from classical xanthines like Theophylline, having a better safety profile.
Pharmacokinetics
Onset
Within 30-60 minutes
Excretion
Excreted mainly via urine as metabolites.
Half life
Approximately 6-8 hours
Absorption
Rapidly absorbed after oral administration, peak plasma levels within 1 hour.
Metabolism
Extensively metabolized in the liver (90%), primarily to inactive metabolites. Not metabolized by CYP450 system.
Side Effects
Contraindications
- Hypersensitivity to doxofylline or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- Lactation
- Severe liver impairment
Drug Interactions
Cimetidine
May increase Doxofylline plasma levels.
Erythromycin
May increase Doxofylline plasma levels.
Phenobarbital, Phenytoin
May decrease Doxofylline plasma levels.
Other xanthine derivatives
Increased risk of toxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe nausea, vomiting, epigastric pain, headache, irritability, insomnia, tachycardia, arrhythmias, and convulsions. Management is symptomatic and supportive; gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy: Use with caution, only if clearly needed and potential benefits outweigh risks. Lactation: Contraindicated, as doxofylline is excreted in breast milk.

Clinical Trials
Doxofylline has been studied in numerous clinical trials demonstrating its efficacy and safety in bronchial asthma and COPD, often showing a better safety profile compared to Theophylline.
Lab Monitoring
- Monitor blood levels of Doxofylline (if therapeutic drug monitoring is available and deemed necessary, though less common for Doxofylline compared to Theophylline).
- Electrolyte levels (especially potassium in cases of overdose or severe adverse reactions).
Doctor Notes
- Consider Doxofylline as an alternative bronchodilator for patients intolerant to Theophylline due to its better safety profile.
- Advise patients on proper administration technique and to report any cardiac symptoms.
- Smoking cessation counseling is crucial for patients with COPD/asthma on this medication.
Patient Guidelines
- Take medicine regularly as prescribed.
- Do not stop taking the medicine without consulting your doctor.
- Report any severe side effects immediately.
- Avoid smoking as it can reduce the effectiveness of the drug.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness or headache in some individuals. Exercise caution when driving or operating machinery until you know how the medicine affects you.
