Overview of the medicine
Doxostar 200 mg Tablet contains Doxycycline, a broad-spectrum tetracycline antibiotic used to treat a wide range of bacterial infections including respiratory, urinary tract, skin, and sexually transmitted infections, as well as malaria prophylaxis.
Uses & Indications
Dosage
Adults
Typically 100-200 mg daily, given as a single dose or divided into two doses (e.g., 100 mg twice daily). Dose and duration depend on the specific infection.
Elderly
No specific dosage adjustment is generally required for elderly patients based on age alone, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
Doxycycline is generally safe for use in patients with renal impairment as it is primarily excreted non-renally. No dosage adjustment is usually necessary.
How to Take
Take Doxostar with a full glass of water, preferably during a meal or with milk to minimize esophageal irritation and gastrointestinal upset. Remain in an upright position (sitting or standing) for at least 30-60 minutes after taking the tablet to prevent esophageal ulceration.
Mechanism of Action
Doxycycline works by inhibiting bacterial protein synthesis. It binds reversibly to the 30S ribosomal subunit of susceptible bacteria, blocking the binding of aminoacyl tRNA to the messenger RNA complex, thereby preventing bacterial growth and replication.
Pharmacokinetics
Onset
Therapeutic effects usually seen within hours to days depending on the infection. Peak plasma concentrations reached within 1-3 hours.
Excretion
Primarily excreted unchanged in the feces via biliary excretion and into the intestinal lumen. A small portion is excreted renally.
Half life
Approximately 16-22 hours, which allows for once or twice daily dosing.
Absorption
Well absorbed after oral administration, with bioavailability typically between 90-100%. Food and dairy products can slightly decrease absorption but are often recommended to reduce GI upset.
Metabolism
Partially metabolized in the liver, but a significant portion is excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to doxycycline or any other tetracycline antibiotic.
- •Children under 8 years of age due to potential for permanent tooth discoloration and enamel hypoplasia.
- •Pregnancy and lactation due to potential for adverse effects on fetal/infant bone and tooth development.
Drug Interactions
Warfarin
Doxycycline may potentiate the anticoagulant effect of warfarin, requiring monitoring of INR and possible dose adjustment.
Oral contraceptives
May reduce the efficacy of oral contraceptives, leading to breakthrough bleeding or unintended pregnancy.
Barbiturates, Phenytoin, Carbamazepine
These drugs can decrease the half-life of doxycycline, potentially reducing its effectiveness.
Antacids (containing aluminum, calcium, magnesium) and Iron preparations
Can significantly reduce the absorption of doxycycline. Administer doxycycline 2-3 hours before or after these agents.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, and diarrhea. Treatment is primarily supportive. Gastric lavage may be considered if ingestion is recent. Doxycycline is not readily removed by hemodialysis.
Pregnancy & Lactation
Doxycycline is contraindicated during pregnancy and lactation. It can cause permanent tooth discoloration (yellow-grey-brown) and enamel hypoplasia in the fetus/infant, as well as reversible inhibition of bone growth.
