Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic effective against a wide range of gram-positive and gram-negative bacteria. It is used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, otitis media, and pharyngitis/tonsillitis.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours for 7-14 days depending on the infection.
Elderly
No dosage adjustment generally required unless severe renal impairment.
Renal_impairment
Dosage adjustment is necessary for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 300 mg daily. For CrCl < 20 mL/min, 200 mg daily.
How to Take
Oral administration, can be taken with or without food. To minimize gastrointestinal upset, it can be taken with food.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, ultimately leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Variable, typically within 1-2 hours for significant plasma levels.
Excretion
Primarily excreted unchanged in urine (about 50%) and bile (about 10%).
Half life
Approximately 3-4 hours.
Absorption
Approximately 30-50% absorbed from the GI tract. Food delays absorption but does not significantly alter the extent.
Metabolism
Not extensively metabolized; about 10% is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or other cephalosporins
- •History of anaphylaxis to penicillin
Drug Interactions
Warfarin
Increased prothrombin time (PT) with or without bleeding, monitor INR.
Probenecid
Increases cefixime plasma levels by decreasing renal excretion.
Carbamazepine
Increased carbamazepine levels reported with concomitant use.
Storage
Store in a cool, dry place, away from direct sunlight. Do not freeze. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. No specific antidote. Symptomatic and supportive treatment is recommended. Hemodialysis and peritoneal dialysis are not effective.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution.
