Description
Overview of the medicine
Desloratadine is a non-drowsy antihistamine used to relieve symptoms of allergic rhinitis (including hay fever) and chronic idiopathic urticaria (hives). It helps to reduce sneezing, runny nose, itchy or watery eyes, and itching of the skin.
Uses & Indications
Dosage
Adults
5 mg (10 mL of 0.5 mg/mL suspension or 5 mL of 1 mg/mL suspension) once daily. For Dysnov 5 mg suspension, typically 5 mL (assuming 1 mg/mL concentration, or 10 mL if 0.5 mg/mL, check product specific label). Assuming 5 mg/5 mL, thus 5 mL once daily for 12+ years.
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with renal or hepatic impairment.
Renal_impairment
For adults and adolescents with renal impairment, a starting dose of 5 mg every other day is recommended.
How to Take
Oral administration. May be taken with or without food. Use an accurate measuring device (e.g., dosing syringe or measuring spoon) to administer the suspension.
Mechanism of Action
Desloratadine is a long-acting, tricyclic antihistamine with selective peripheral H1-receptor antagonist activity. It prevents histamine from binding to H1-receptors, thereby inhibiting the effects of histamine such as vasodilation, increased capillary permeability, and stimulation of sensory nerves.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Excreted in urine (approx. 41%) and feces (approx. 47%) as metabolites.
Half life
Approximately 27 hours (terminal elimination half-life).
Absorption
Rapidly absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached approximately 3 hours after dosing.
Metabolism
Extensively metabolized, primarily to 3-hydroxydesloratadine, a major active metabolite. It is metabolized by glucuronidation and hydroxylation.
Side Effects
Contraindications
- Hypersensitivity to desloratadine, loratadine, or any component of the formulation.

- Generally, no specific laboratory monitoring is required for desloratadine. In patients with hepatic or renal impairment, liver and kidney function tests may be monitored if clinically indicated.
Doctor Notes
- Counsel patients on proper administration technique for suspension, especially for pediatric dosing.
- Advise patients that while generally non-sedating, individual responses can vary; caution should be exercised with activities requiring mental alertness.
- Monitor patients with severe renal or hepatic impairment for potential dose adjustments.
Patient Guidelines
- Follow your doctor's instructions regarding dosage and duration of treatment.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
- If symptoms do not improve or worsen, consult your doctor.
