Description
Overview of the medicine
Dyvon ointment/gel/cream is a topical medication used for the treatment of plaque psoriasis. It combines calcipotriol, a vitamin D analogue that helps regulate skin cell growth, and betamethasone dipropionate, a potent corticosteroid that reduces inflammation and itching.
Uses & Indications
Dosage
Adults
Apply to affected areas once daily. Max 15g per day, max 100g per week. Max treatment duration 4 weeks (body) or 8 weeks (scalp).
Elderly
No special dosage adjustment needed.
Renal_impairment
Use with caution in patients with severe renal or hepatic impairment due to potential systemic effects.
How to Take
For external use only. Apply a thin layer to the affected skin areas. Do not apply to the face, eyes, or mucous membranes. Wash hands after application.
Mechanism of Action
Calcipotriol is a synthetic vitamin D3 derivative that inhibits keratinocyte proliferation and promotes their differentiation. Betamethasone dipropionate is a potent corticosteroid that exerts anti-inflammatory, antipruritic, and vasoconstrictive actions through its binding to glucocorticoid receptors and subsequent modulation of gene expression.
Pharmacokinetics
Onset
Symptomatic improvement typically within 2 weeks, with optimal effects by 8 weeks.
Excretion
Mainly excreted in feces (calcipotriol metabolites) and urine (betamethasone metabolites).
Half life
Calcipotriol: < 1 hour (systemic). Betamethasone: ~5.6 hours (systemic after absorption).
Absorption
Systemic absorption is minimal but can increase with prolonged use, large surface areas, or occlusive dressings. Calcipotriol absorption is around 5-6%, and betamethasone absorption is less than 1%.
Metabolism
Calcipotriol is metabolized in the liver to inactive metabolites. Betamethasone is metabolized primarily in the liver.
Side Effects
Contraindications
- Hypersensitivity to calcipotriol, betamethasone, or any excipients
- Erythrodermic, exfoliative, and pustular psoriasis
- Impaired calcium metabolism
- Viral (e.g., herpes, varicella), fungal, bacterial, or parasitic skin infections
- Skin atrophy, striae, fragility of skin veins, ichthyosis, acne vulgaris, acne rosacea, perioral dermatitis, ulcers, wounds
- Children under 18 years
Drug Interactions
Other topical corticosteroids
Avoid concurrent use as it may increase systemic absorption and adverse effects.
Calcium metabolism affecting agents
Use with caution with other agents known to affect calcium metabolism to prevent hypercalcemia.
Storage
Store below 30°C. Do not refrigerate or freeze. Keep out of reach of children.
Overdose
Excessive use may lead to hypercalcaemia (due to calcipotriol) and suppression of the hypothalamic-pituitary-adrenal (HPA) axis (due to betamethasone). Treatment is symptomatic and supportive; discontinue the drug.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: Use with caution. Avoid applying to the breast area.
