Description
Overview of the medicine
e-proton (Esomeprazole) is a proton pump inhibitor used to treat conditions caused by too much stomach acid, such as GERD, ulcers, and Zollinger-Ellison syndrome. It works by reducing the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
Typically 20-40 mg once daily, depending on the condition. For GERD: 20 mg once daily for 4-8 weeks. For erosive esophagitis: 20-40 mg once daily for 4-8 weeks.
Elderly
No specific dosage adjustment usually needed, but caution advised due to potential for age-related decline in renal or hepatic function.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take whole capsule at least one hour before a meal. Do not chew or crush. For patients with difficulty swallowing, the capsule can be opened and the pellets mixed with a small amount of water or fruit juice, then swallowed immediately.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby blocking the final step in acid secretion. This leads to a reduction in gastric acid output.

Within 1 hour.
Excretion
Primarily via urine (80%) and feces (20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, particularly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin absorption; monitor digoxin levels.
Diazepam
Decreased clearance of diazepam; dose adjustment may be necessary.
Warfarin
Increased INR/prothrombin time; monitoring is advised.
Clopidogrel
Reduced antiplatelet effect; consider alternative antiplatelet therapy or PPI.
Methotrexate
Increased methotrexate levels, particularly with high doses; monitor for toxicity.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include blurred vision, confusion, drowsiness, flushing, headache, nausea, tachycardia, and vomiting. Treatment is symptomatic and supportive, as there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Esomeprazole is excreted in breast milk; caution advised when administering to a nursing mother.
