Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions like heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on the indication. For erosive esophagitis, 40 mg once daily for 4-8 weeks. For H. pylori eradication, 40 mg once daily for 10 days with appropriate antibiotics.
Elderly
No specific dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole with water, at least one hour before a meal, preferably in the morning. Do not chew or crush the capsule. If swallowing is difficult, the capsule can be opened and the pellets mixed with a small amount of water or fruit juice and swallowed immediately.
Mechanism of Action
Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (the proton pump) on the secretory surface of the gastric parietal cells. This action effectively blocks the final step in gastric acid production, leading to a sustained decrease in acid secretion.
Pharmacokinetics
Onset
Acid inhibition begins within 1 hour of oral administration.
Excretion
Approximately 80% of the dose is excreted as metabolites in the urine, and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1-2 hours. Bioavailability is approximately 64% after a single 40 mg dose.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any components of the formulation
- •Concomitant use with nelfinavir (an antiretroviral drug)
Drug Interactions
Warfarin
Potential for increased INR and bleeding risk.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
May increase serum levels of methotrexate, particularly with high doses.
Nelfinavir/Atazanavir
Decreased plasma levels of nelfinavir and atazanavir due to reduced absorption.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include gastrointestinal disturbances and weakness. Treatment is symptomatic and supportive. Esomeprazole is not readily dialyzable.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is unknown if esomeprazole is excreted in human milk; use with caution during lactation.
