Description
Overview of the medicine
Ebastine is a non-sedating, long-acting H1-antihistamine used to treat allergic conditions such as seasonal and perennial allergic rhinitis, and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
10 mg once daily. In severe cases or for chronic idiopathic urticaria, the dose may be increased to 20 mg once daily.
Elderly
No special dosage adjustment is usually required in elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is required in patients with renal impairment.
How to Take
Ebatin-10-mg-tablet is for oral administration. It can be taken with or without food, preferably at the same time each day.
Mechanism of Action
Ebastine selectively blocks peripheral H1-histamine receptors, preventing histamine-induced allergic symptoms like sneezing, runny nose, watery eyes, and itching. It does not readily cross the blood-brain barrier, contributing to its non-sedating profile.
Pharmacokinetics
Onset
Onset of action for symptomatic relief typically occurs within 1-3 hours.
Excretion
Primarily excreted via urine and feces as metabolites, with less than 10% excreted unchanged.
Half life
The plasma half-life of carebastine is approximately 15-19 hours.
Absorption
Rapidly absorbed from the GI tract after oral administration. Undergoes extensive first-pass metabolism in the liver to its active metabolite, carebastine.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 3A4 (CYP3A4) isoenzyme to its active metabolite, carebastine.
Side Effects
Contraindications
- •Hypersensitivity to ebastine or to any of the excipients of the formulation.
- •Severe hepatic impairment.
Drug Interactions
Rifampicin
May decrease plasma levels of ebastine and carebastine due to induction of CYP3A4, reducing efficacy.
Other antihistamines
Concurrent use is generally not recommended as it may increase side effects without additional benefit.
Ketoconazole, Erythromycin, Clarithromycin
Concomitant use may lead to increased plasma levels of ebastine and its active metabolite, carebastine, potentially increasing the risk of QT interval prolongation. Close monitoring is recommended.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include CNS depression (e.g., drowsiness) or excitation (e.g., restlessness, anxiety, tachycardia). In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage and activated charcoal may be considered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Animal studies have not shown adverse effects, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Not recommended during breastfeeding as ebastine or its metabolite may be excreted in breast milk.
