Description
Overview of the medicine
Ecena (Esomeprazole) is a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions like heartburn, gastroesophageal reflux disease (GERD), and stomach ulcers.
Uses & Indications
Dosage
Adults
GERD: 20 mg or 40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 10 days with appropriate antibiotics. Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function. However, caution should be exercised in severe cases.
How to Take
Oral tablets/capsules should be swallowed whole with water at least one hour before a meal. Do not chew or crush. For patients unable to swallow, oral suspension or IV formulation may be used.
Mechanism of Action
Esomeprazole is an S-isomer of Omeprazole. It inhibits the H+/K+-ATPase enzyme system (proton pump) at the secretory surface of the gastric parietal cell, thereby blocking the final step of acid production.

Approximately 80% of metabolites are excreted in the urine and 20% in the feces.

Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration, peak plasma levels occur within 1-2 hours. Bioavailability is approximately 64% after a single 40 mg dose and increases to 89% after repeated daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with Nelfinavir.
Drug Interactions
Digoxin
Increased Digoxin exposure.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Reduced antiplatelet effect of Clopidogrel due to CYP2C19 inhibition.
Methotrexate
May increase Methotrexate levels, particularly at high doses.
Atazanavir, Nelfinavir
Reduced absorption and plasma levels of these antiretrovirals.
Storage
Store below 30°C, in a dry place, away from light. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, and headache. No specific antidote is known. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
