Description
Overview of the medicine
Ecox 60 mg tablet contains Etoricoxib, a non-steroidal anti-inflammatory drug (NSAID) that selectively inhibits cyclooxygenase-2 (COX-2). It is used to relieve pain and inflammation in various conditions such as osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis.
Uses & Indications
Dosage
Adults
Osteoarthritis: 60 mg once daily. Rheumatoid Arthritis, Ankylosing Spondylitis: 90 mg once daily (Ecox 60 mg is not typically prescribed for this, higher strength might be needed). Acute Gouty Arthritis: 120 mg once daily for a maximum of 8 days. Acute pain, Primary Dysmenorrhea, Post-dental surgery pain: 120 mg once daily for a maximum of 8 days or 90 mg once daily for max 3 days, respectively (higher strength needed for these indications).
Elderly
No dosage adjustment is generally required for elderly patients, but caution is advised. Start with the lowest effective dose.
Renal_impairment
Mild renal impairment (CrCl 30-80 mL/min): No dosage adjustment. Severe renal impairment (CrCl <30 mL/min): Not recommended.
How to Take
Ecox 60 mg tablet should be taken orally once daily. It can be taken with or without food. For faster onset of action, it may be taken without food.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins, which are mediators of pain and inflammation. By inhibiting COX-2, etoricoxib reduces prostaglandin production, thereby alleviating pain and inflammation.

Onset
Pain relief may begin within 24 minutes after a single dose for acute conditions.
Excretion
Approximately 70% excreted in urine (as metabolites) and 20% in faeces (as metabolites and unchanged drug).
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations (Tmax) achieved in approximately 1 hour. Oral bioavailability is about 100%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any component of the tablet
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic dysfunction
- •Severe renal impairment (CrCl <30 mL/min)
- •Congestive heart failure (NYHA II-IV)
- •Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Uncontrolled hypertension
- •Pregnancy and lactation
Drug Interactions
Digoxin
May increase digoxin plasma levels.
Lithium
May increase lithium plasma levels and toxicity.
Warfarin
May increase INR and risk of bleeding.
Diuretics
May reduce natriuretic effect.
Rifampicin
May decrease etoricoxib plasma concentrations.
Methotrexate
May increase methotrexate toxicity.
Oral Contraceptives
May increase plasma concentrations of ethinyl estradiol.
ACE Inhibitors/Angiotensin Receptor Blockers
May reduce antihypertensive effect and increase risk of renal impairment.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, symptomatic and supportive measures should be initiated. Etoricoxib is not dialyzable. Monitor vital signs and provide general supportive care.
Pregnancy & Lactation
Etoricoxib is contraindicated during pregnancy, especially in the third trimester, due to potential risks to the fetus (e.g., premature closure of the ductus arteriosus). It is also not recommended during lactation as it is unknown if etoricoxib is excreted in human milk.
