Ivermectin is an anti-parasitic medication used to treat certain parasitic infections in humans, including strongyloidiasis, onchocerciasis, scabies, and pediculosis.
Uses & Indications
Dosage
Adults
Strongyloidiasis: 200 mcg/kg as a single oral dose. Onchocerciasis: 150 mcg/kg as a single oral dose every 3-12 months. Scabies: 200 mcg/kg as a single oral dose, repeated after 7-14 days if needed.
Elderly
No specific dosage adjustment is generally required unless hepatic or renal impairment is present. Use with caution due to potential age-related decrease in organ function.
Renal_impairment
Use with caution in patients with severe renal impairment (CrCl <30 mL/min); dosage adjustment may be necessary. Not dialyzable.
How to Take
Administer orally with water. For Strongyloidiasis and Scabies, it may be taken with food to enhance absorption. For Onchocerciasis, it is typically taken on an empty stomach to reduce variability.
Mechanism of Action
Ivermectin binds selectively and with high affinity to glutamate-gated chloride ion channels, which occur in invertebrate nerve and muscle cells. This binding leads to an increase in the permeability of the cell membrane to chloride ions, resulting in hyperpolarization of the nerve or muscle cell, leading to paralysis and death of the parasite.

Drug Interactions
Warfarin
Ivermectin may potentiate the anticoagulant effect of warfarin. Monitor INR closely.
CNS depressants (e.g., benzodiazepines, barbiturates, alcohol)
May enhance CNS depressant effects, increasing risk of dizziness or drowsiness.
