Overview of the medicine
Ivermectin is an anti-parasitic medication used to treat certain parasitic infections, including strongyloidiasis, onchocerciasis, and scabies. It belongs to the avermectin class of drugs.
Uses & Indications
Dosage
Adults
For strongyloidiasis: A single oral dose of 200 mcg/kg body weight. For onchocerciasis: A single oral dose of 150 mcg/kg body weight, repeated every 6-12 months. For scabies: A single oral dose of 200 mcg/kg body weight, repeated after 7-14 days.
Elderly
No specific dosage adjustment required for elderly patients based solely on age, but consider overall health, renal, and hepatic function.
Renal_impairment
Use with caution in severe renal impairment. No specific dose adjustment guidelines are established for mild to moderate impairment, but monitoring is advised.
How to Take
Administer orally with water on an empty stomach, usually 30 minutes before food. This helps to ensure optimal absorption.
Mechanism of Action
Ivermectin binds selectively and with high affinity to glutamate-gated chloride ion channels which occur in invertebrate nerve and muscle cells. This leads to an increase in the permeability of the cell membrane to chloride ions, resulting in hyperpolarization of the nerve or muscle cell, and subsequent paralysis and death of the parasite.

Onset of action varies depending on the parasitic infection, often within days to weeks for therapeutic effects.
Excretion
Primarily excreted in feces (approximately 98%), almost exclusively as metabolites. Less than 1% of the administered dose is excreted in urine.
Half life
The plasma elimination half-life is approximately 16 hours for a single 12 mg oral dose.
Absorption
Well absorbed orally. Peak plasma concentrations are reached approximately 4 hours after administration. Bioavailability is increased when taken with a fatty meal.
Metabolism

Small amounts are excreted in breast milk; caution is advised, or avoid breastfeeding for 12 hours after treatment to minimize infant exposure.
