Description
Overview of the medicine
Elocef 125 mg Suspension is an antibiotic medication containing Cefpodoxime Proxetil, belonging to the third-generation cephalosporin class. It is used to treat a wide range of bacterial infections in children and adults.
Uses & Indications
Dosage
Adults
Dosage varies by infection. E.g., for pharyngitis/tonsillitis: 100 mg every 12 hours for 5-10 days; for community-acquired pneumonia: 200 mg every 12 hours for 14 days. This 125mg suspension is primarily for pediatric use.
Elderly
Dosage may need adjustment based on renal function, similar to adults with renal impairment.
Renal_impairment
Dosage adjustment is necessary. For CrCl <30 mL/min, administer standard dose every 24 hours. For CrCl <10 mL/min, administer standard dose every 48 hours.
How to Take
Administer orally. The suspension can be taken with or without food. Shake well before use. Use a calibrated measuring device for accurate dosing, especially for children.
Mechanism of Action
Cefpodoxime Proxetil is a prodrug that is hydrolyzed to Cefpodoxime. Cefpodoxime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to lysis of the bacterial cell.
Pharmacokinetics
Onset
Within 1-3 hours (peak plasma concentration)
Excretion
Primarily excreted unchanged in the urine (about 29-38% of the dose) within 12 hours.
Half life
Approximately 2-3 hours
Absorption
Rapidly absorbed from the gastrointestinal tract and hydrolyzed to Cefpodoxime. Food enhances absorption.
Metabolism
Metabolized to its active form, Cefpodoxime; minimal hepatic metabolism of Cefpodoxime itself.
Side Effects
Contraindications
- Known hypersensitivity to Cefpodoxime, other cephalosporins, or any component of the formulation
- Patients with severe hypersensitivity to penicillin should also avoid due to potential cross-reactivity.
Drug Interactions
Probenecid
Increases plasma concentration and duration of action of cefpodoxime by inhibiting renal excretion.
Anticoagulants (e.g., warfarin)
May enhance the anticoagulant effect; monitor INR.
Nephrotoxic drugs (e.g., aminoglycosides)
Increased risk of nephrotoxicity with concomitant use.
Antacids/H2 blockers (e.g., ranitidine, cimetidine)
May decrease cefpodoxime absorption, leading to lower plasma concentrations. Administer cefpodoxime at least 2 hours before or after antacids.
Storage
Store dry powder below 30°C (86°F), protected from moisture. Store reconstituted suspension in a refrigerator (2-8°C or 36-46°F); do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. In case of overdose, symptomatic and supportive treatment is recommended. Hemodialysis and peritoneal dialysis may aid in removal from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm, but human studies are lacking. Use only if clearly needed. Cefpodoxime is excreted in small amounts in breast milk; use with caution in nursing mothers.
