Description
Overview of the medicine
Escitalopram is an antidepressant used to treat major depressive disorder, generalized anxiety disorder, panic disorder, social anxiety disorder, and obsessive-compulsive disorder. It is a selective serotonin reuptake inhibitor (SSRI).
Uses & Indications
Dosage
Adults
Initially 10 mg once daily, may be increased to a maximum of 20 mg once daily after at least one week. Doses above 10 mg should be individualized.
Elderly
Initially 5 mg once daily; maximum 10 mg once daily.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl < 30 mL/min).
How to Take
May be taken with or without food, preferably at the same time each day.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Therapeutic effects typically observed after 2-4 weeks of treatment.
Excretion
Primarily excreted via urine, with some fecal excretion.
Half life
Approximately 27-32 hours.
Absorption
Well absorbed orally. Peak plasma concentration reached in approximately 4-5 hours. Bioavailability is about 80%.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, CYP2C19, and CYP2D6 to its active and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or citalopram
- •Concomitant use with MAOIs (monoamine oxidase inhibitors) or within 14 days of discontinuing an MAOI
- •Concomitant use with pimozide
- •Congenital long QT syndrome or known acquired long QT interval prolongation
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes). Avoid concomitant use.
Pimozide
Contraindicated due to risk of QT prolongation.
Warfarin
Increased risk of bleeding due to potential potentiation of anticoagulant effect.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
Tricyclic Antidepressants (TCAs)/Lithium
Increased plasma levels of TCAs/lithium, monitor for toxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, tremor, agitation, somnolence, nausea, vomiting, tachycardia, hypotension, and rarely, seizures or serotonin syndrome. Treatment is supportive; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Neonates exposed to SSRIs late in the third trimester have developed complications requiring prolonged hospitalization, respiratory support, and tube feeding. It is excreted in human milk; generally not recommended during breastfeeding.
