Description
Overview of the medicine
Elodep 5 mg tablet contains escitalopram, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, generalized anxiety disorder, social anxiety disorder, panic disorder, and obsessive-compulsive disorder.
Uses & Indications
Dosage
Adults
The usual starting dose for most conditions is 10 mg once daily. For certain conditions or individuals, a starting dose of 5 mg once daily may be appropriate. The dose can be increased to a maximum of 20 mg once daily after at least one week.
Elderly
A lower starting dose of 5 mg once daily is recommended for elderly patients (over 65 years). Maximum dose should generally not exceed 10 mg daily.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Caution is advised in severe renal impairment, and dosage should be determined by a physician.
How to Take
Elodep 5 mg tablet should be taken orally once daily, with or without food. Swallow the tablet whole with a glass of water; do not chew or crush it. It is recommended to take it at the same time each day.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) by neurons, leading to an increase in serotonin concentration in the synaptic cleft. This potentiation of serotonergic neurotransmission is thought to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Therapeutic effects typically begin within 2-4 weeks of continuous treatment, although some patients may experience improvement sooner.
Excretion
Excreted primarily through the urine (approximately 8% as unchanged drug) and feces.
Half life
The elimination half-life is approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed after oral administration, bioavailability is approximately 80%. Peak plasma concentration (Cmax) is reached in about 4 hours.
Metabolism
Extensively metabolized in the liver primarily by CYP2C19, CYP3A4, and CYP2D6 enzymes into inactive or less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or any excipients.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- •Concomitant use with pimozide, linezolid, or intravenous methylene blue.
- •Congenital long QT syndrome or a history of QT prolongation.
Drug Interactions
MAOIs
Concomitant use is contraindicated due to risk of serotonin syndrome.
Pimozide
Concomitant use is contraindicated due to potential for QT prolongation.
Triptans
Increased risk of serotonin syndrome.
Warfarin
May increase anticoagulant effect; monitor INR.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
St. John's Wort
May increase serotonergic effects and risk of adverse reactions.
QT-prolonging drugs
Increased risk of QT prolongation and arrhythmia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of escitalopram overdose include dizziness, tremor, somnolence, insomnia, nausea, vomiting, tachycardia, hypotension, and rarely, seizures or serotonin syndrome. Management is primarily supportive; maintain airway, monitor cardiac rhythm and vital signs. Activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy Category C/D (depending on trimester, potential for Persistent Pulmonary Hypertension of the Newborn (PPHN) if used in late pregnancy). Use only if potential benefit outweighs potential risk to the fetus. Escitalopram is excreted in human milk; use with caution during lactation, and consult a physician.
