Overview of the medicine
Emitof (Esomeprazole) is a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is prescribed for conditions like gastroesophageal reflux disease (GERD), erosive esophagitis, and ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily for 7-10 days (with antibiotics).
Elderly
No specific dose adjustment is typically required based on age.
Renal_impairment
No dose adjustment is needed for patients with impaired renal function.
How to Take
Take at least one hour before a meal. Swallow the capsule whole with water. Do not chew or crush. For patients who have difficulty swallowing, the capsule can be opened and the pellets mixed with a small amount of water or soft food.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This action prevents the final step of acid secretion, thereby reducing gastric acidity.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Excreted mainly by the kidneys (approximately 80%) as metabolites, and the remaining by fecal route (approximately 20%).
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 1-2 hours. Bioavailability is about 64% after a single 40 mg dose.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily via CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased bioavailability of digoxin.
Diazepam
Decreased metabolism of diazepam.
Warfarin
Increased INR and prothrombin time.
Tacrolimus
Increased plasma levels of tacrolimus.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma concentrations of methotrexate.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Experience with esomeprazole overdose is limited. Symptoms may include drowsiness, tachycardia, and abdominal pain. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Caution should be exercised when Emitof is administered to a nursing mother.
