Overview of the medicine
Empagliflozin is an oral antidiabetic drug used to treat type 2 diabetes mellitus, heart failure, and chronic kidney disease. It works by inhibiting SGLT2 in the kidneys, leading to increased glucose excretion in urine.
Uses & Indications
Dosage
Adults
Type 2 Diabetes: 10 mg once daily, may be increased to 25 mg once daily. Heart Failure/CKD: 10 mg once daily.
Elderly
No dosage adjustment required based on age alone, but renal function should be assessed.
Renal_impairment
Not recommended if eGFR < 20 mL/min/1.73m². Dosage adjustment may not be needed for eGFR ≥ 20 mL/min/1.73m².
How to Take
Take orally once daily in the morning, with or without food.
Mechanism of Action
Empagliflozin selectively inhibits sodium-glucose cotransporter 2 (SGLT2) in the renal proximal tubules, reducing glucose reabsorption and increasing urinary glucose excretion. This leads to a reduction in blood glucose levels, along with osmotic diuresis, natriuresis, and weight loss.

Approximately half is excreted in urine and half in feces.

Approximately 12.4 hours.
Absorption

Side Effects
Contraindications
- •Hypersensitivity to empagliflozin or any excipients.
- •Severe renal impairment (eGFR < 20 mL/min/1.73m²), end-stage renal disease, or dialysis.
Drug Interactions
Diuretics
May increase risk of dehydration and hypotension.
Insulin and Insulin Secretagogues
Increased risk of hypoglycemia (dose adjustment may be needed).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to increased urinary glucose excretion, dehydration, and hypotension. Management involves symptomatic and supportive measures.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding due to potential risk to the infant.
