Description
Overview of the medicine
Empagliflozin is an oral antidiabetic medication. It is used to lower blood sugar levels in adults with type 2 diabetes mellitus. It also reduces the risk of cardiovascular death, hospitalization for heart failure, and progression of kidney disease in certain patients.
Uses & Indications
Dosage
Adults
10 mg once daily in the morning, with or without food. May be increased to 25 mg once daily if tolerated and additional glycemic control is needed.
Elderly
No specific dose adjustment required based on age alone. Consider renal function.
Renal_impairment
Initial dose 10 mg once daily. Not recommended for eGFR < 20 mL/min/1.73m².
How to Take
Oral. Take once daily in the morning, with or without food.
Mechanism of Action
Empagliflozin selectively inhibits the sodium-glucose co-transporter 2 (SGLT2) in the renal tubules, reducing glucose reabsorption and increasing glucose excretion in the urine, thereby lowering blood glucose levels.

Onset
Glycemic effect observed within hours, but full benefits over time.
Excretion
Approximately 50% renally excreted and 50% hepatobiliary/fecally excreted.
Half life
Approximately 10-13 hours.
Absorption
Rapidly absorbed. Peak plasma concentration (Cmax) reached within 1.5 hours. Bioavailability is approximately 78%.
Metabolism
Primarily metabolized by glucuronidation by UGT1A3, UGT1A8, UGT1A9, and UGT2B7.
Side Effects
Contraindications
- •Hypersensitivity to empagliflozin or any excipients
- •Severe renal impairment (eGFR < 20 mL/min/1.73m²)

Drug Interactions
Diuretics
Increased risk of dehydration and hypotension.
ACE Inhibitors/ARBs
Increased risk of hypotension in some patients.
Insulin and Insulin Secretagogues
Increased risk of hypoglycemia (dose adjustment may be needed).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek medical attention. Symptomatic treatment and supportive measures should be initiated. Empagliflozin is not expected to be dialyzable.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy due to potential for adverse renal development. Avoid during lactation as it may be excreted in breast milk.
