Enar 500 mg Tablet is a synthetic broad-spectrum antibiotic from the fluoroquinolone class, primarily used to treat various bacterial infections throughout the body.
Uses & Indications
Dosage
Adults
Typically 500 mg orally twice daily for 7 to 14 days, depending on the severity and type of infection. Dosage and duration may vary.
Elderly
No specific dosage adjustment is required for elderly patients with normal renal function. Dosage should be adjusted based on renal impairment.
Renal_impairment
For creatinine clearance <30 mL/min, reduce the dose (e.g., 250-500 mg once daily) or increase the dosing interval. Consult a doctor for specific guidance.
How to Take
Enar tablets should be swallowed whole with a glass of water, with or without food. Do not chew or crush the tablet. It should not be taken with dairy products or calcium-fortified juices, or antacids.
Mechanism of Action
Enar works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to cell death of susceptible bacteria.

Excretion
Primarily excreted renally (approximately 50-70% as unchanged drug) and to a lesser extent via feces.
Half life
Approximately 4 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal (GI) tract after oral administration. Peak plasma concentrations are typically reached within 1-2 hours.
Metabolism
Partially metabolized in the liver via the cytochrome P450 system, forming several metabolites, some of which have antimicrobial activity.
Side Effects
Contraindications
- Hypersensitivity to Enar or other fluoroquinolones.
- Concomitant administration with tizanidine.
- History of tendinitis or tendon rupture associated with quinolone use.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. Monitor INR regularly.
Tizanidine
Concomitant use is contraindicated due to a significant increase in tizanidine plasma concentrations, leading to hypotension and sedation.
Theophylline
May increase theophylline levels, leading to toxicity. Monitor theophylline levels.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May increase the risk of CNS stimulation and convulsions when used concurrently with Enar.
Antacids (Magnesium/Aluminum), Sucralfate, Didanosine
Can significantly reduce Enar absorption. Administer Enar at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive treatment should be initiated. Gastric lavage and activated charcoal may be used shortly after ingestion. Hemodialysis or peritoneal dialysis is not effective in removing Enar from the body.
Pregnancy & Lactation
Enar should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk and is not recommended during breastfeeding due to potential risk of serious adverse reactions in the infant.
