Description
Overview of the medicine
Endamycin is a macrolide antibiotic used to treat a variety of bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
150 mg once daily or 75 mg twice daily, depending on the severity and type of infection. Treatment duration is typically 5-10 days.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with hepatic or renal impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl < 30 mL/min), consider reducing the dose or extending the dosing interval.
How to Take
Take the capsule orally, with or without food. To minimize gastrointestinal upset, it may be taken with a meal. Swallow the capsule whole with water; do not crush or chew.
Mechanism of Action
Endamycin acts by inhibiting bacterial protein synthesis through binding to the 50S ribosomal subunit, thereby preventing translocation of peptide chains. This primarily results in a bacteriostatic effect.
Pharmacokinetics
Onset
Peak plasma concentrations are typically reached within 2-3 hours.
Excretion
Mainly excreted in the bile and feces; a small portion is excreted via urine.
Half life
Approximately 8-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, though bioavailability may be reduced with food.
Metabolism
Primarily metabolized in the liver via the cytochrome P450 system (CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to endamycin or any other macrolide antibiotics
- Severe hepatic dysfunction
- Concurrent use with drugs that prolong the QT interval (e.g., cisapride, terfenadine)
Drug Interactions
Digoxin
May increase serum digoxin levels.
Warfarin
May increase anticoagulant effect, requiring INR monitoring.
Theophylline
May increase theophylline concentrations, leading to toxicity.
CYP3A4 Inhibitors
Co-administration with potent CYP3A4 inhibitors (e.g., ketoconazole) may increase endamycin levels.
Antacids containing Magnesium or Aluminum
May reduce the absorption of endamycin.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Symptomatic and supportive measures should be employed. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Endamycin is excreted in breast milk; caution should be exercised when administered to a nursing mother.
