Overview of the medicine
Enigma 50 mg Tablet contains Serenagene Hydrochloride, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, anxiety disorders, and obsessive-compulsive disorder. It works by increasing the levels of serotonin in the brain, a natural substance that helps maintain mental balance.
Uses & Indications
Dosage
Adults
Initially 50 mg once daily, preferably in the morning or evening. Dosage may be increased to 100 mg after several weeks if needed, based on clinical response and tolerability. Maximum recommended dose is 200 mg daily.
Elderly
A lower starting dose (e.g., 25 mg) and slower titration may be considered. Maximum recommended dose 100 mg daily.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl <30 mL/min).
How to Take
Take Enigma tablet orally, once daily, with or without food. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Serenagene Hydrochloride selectively inhibits the reuptake of serotonin by presynaptic neurons, thereby increasing the concentration of serotonin in the synaptic cleft. This enhanced serotonergic neurotransmission is thought to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial effects may be seen within 1-2 weeks, but full antidepressant effect may take 4-6 weeks.
Excretion
Mainly excreted in urine (approximately 60%) and feces (approximately 35%) as metabolites.
Half life
Approximately 20-26 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with peak plasma concentrations reached within 4-8 hours. Bioavailability is approximately 80%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Serenagene Hydrochloride or any excipients
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment
- •Concomitant use with pimozide
- •Uncontrolled narrow-angle glaucoma
Drug Interactions
MAOIs
Concurrent use may lead to serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes). A washout period of at least 14 days is required.
NSAIDs
Increased risk of gastrointestinal bleeding when co-administered.
Triptans
Increased risk of serotonin syndrome when used with other serotonergic drugs like triptans.
Warfarin
May increase the anticoagulant effect of warfarin, requiring careful monitoring of INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, tachycardia, tremor, agitation, and rarely, convulsions or serotonin syndrome. Treatment is symptomatic and supportive. Maintain an open airway, monitor cardiac rhythm and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, use with caution in breastfeeding mothers, weighing the benefits against potential risks to the infant.
