Description
Overview of the medicine
Enoparin 4000 anti-Xa injection contains Enoxaparin Sodium, a low molecular weight heparin (LMWH), used to prevent and treat various thromboembolic disorders such as deep vein thrombosis (DVT) and pulmonary embolism (PE). It works by inhibiting specific clotting factors in the blood.
Uses & Indications
Dosage
Adults
For DVT prophylaxis: 40 mg (4000 anti-Xa IU) once daily SC. For DVT/PE treatment: 1 mg/kg (100 anti-Xa IU/kg) every 12 hours SC, or 1.5 mg/kg (150 anti-Xa IU/kg) once daily SC.
Elderly
No dose adjustment usually required for age, but monitor renal function carefully.
Renal_impairment
Severe renal impairment (CrCl < 30 mL/min) requires dose adjustment (e.g., 20 mg once daily for prophylaxis, 1 mg/kg once daily for treatment).
How to Take
Administer by deep subcutaneous injection into the anterolateral or posterolateral abdominal wall, alternating between the left and right sides. Do not inject into the muscle. Do not expel the air bubble from the syringe before injection.
Mechanism of Action
Enoxaparin selectively potentiates the inhibitory effect of antithrombin III on Factor Xa, and to a lesser extent, on Factor IIa (thrombin). This results in anticoagulant activity by preventing clot formation.

Excretion
Renal elimination (around 40% as active or partially active fragments).

Approximately 4.5 to 7 hours after a single SC dose.
Absorption
Rapid and complete after subcutaneous (SC) injection. Bioavailability is approximately 100%.
Metabolism
Primarily desulfation and depolymerization in the liver to lower molecular weight species with reduced biological potency.
Side Effects
Contraindications
- Active major bleeding.
- History of heparin-induced thrombocytopenia (HIT) with or without thrombosis.
- Hypersensitivity to enoxaparin, heparin, or pork products.
- Acute bacterial endocarditis.
- Severe uncontrolled hypertension.

Increased risk of bleeding (careful monitoring required when transitioning).
