Description
Overview of the medicine
Enoparin-6000-anti-Xa-injection is a low molecular weight heparin (LMWH) containing enoxaparin sodium, used to prevent and treat various thromboembolic disorders. It works by inhibiting coagulation factors, primarily Factor Xa.
Uses & Indications
Dosage
Adults
DVT prophylaxis: 40 mg SC once daily or 30 mg SC every 12 hours. DVT/PE treatment: 1 mg/kg SC every 12 hours or 1.5 mg/kg SC once daily.
Elderly
Dose reduction may be considered, especially in renal impairment. Monitor anti-Xa levels if indicated.
Renal_impairment
Severe renal impairment (CrCl <30 mL/min) requires dose adjustment (e.g., 30 mg SC once daily for prophylaxis, 1 mg/kg SC once daily for treatment).
How to Take
Administered via subcutaneous (SC) injection into the abdominal wall. Do not inject intramuscularly. Alternate injection sites.
Mechanism of Action
Enoxaparin binds to antithrombin III (ATIII), increasing its affinity for Factor Xa, and to a lesser extent, Factor IIa (thrombin). This binding inhibits these clotting factors, primarily Factor Xa, preventing the formation of fibrin clots.
Pharmacokinetics
Onset
3-5 hours (peak effect)
Excretion
Primarily renal elimination of active and inactive fragments.
Half life
Approximately 4.5 to 7 hours after a single SC dose.
Absorption
Rapid and complete after subcutaneous (SC) administration. Peak anti-Xa activity achieved in 3-5 hours.
Metabolism
Primarily desulfation and depolymerization in the liver to less active metabolites.
Side Effects
Contraindications
- Active major bleeding or conditions with high risk of uncontrolled bleeding (e.g., hemorrhagic stroke, recent surgery with active bleeding).
- History of heparin-induced thrombocytopenia (HIT) with or without thrombosis.
- Hypersensitivity to enoxaparin, heparin, or porcine products.
- Spinal/epidural anesthesia or lumbar puncture in patients receiving therapeutic anticoagulation with enoxaparin due to risk of spinal hematoma.
Drug Interactions
Dextran
Increased risk of bleeding.
NSAIDs, antiplatelet agents (e.g., aspirin, clopidogrel), other anticoagulants (e.g., warfarin, dabigatran)
Increased risk of bleeding.
Storage
Store below 25°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Accidental overdose may lead to hemorrhagic complications. The anticoagulant effects can be largely neutralized by the slow intravenous injection of protamine sulfate.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy when clinically indicated. Limited data on excretion in breast milk; generally considered compatible with breastfeeding with caution. Consult a doctor.

Lab Monitoring
- Complete Blood Count (CBC) including platelet count (before and during therapy).
- Renal function tests (e.g., creatinine clearance).
- Anti-Xa levels (may be monitored in specific populations like renal impairment, obesity, pregnancy).
- Stool occult blood tests.
Doctor Notes
- Always assess bleeding risk factors before initiating enoxaparin.
- Consider anti-Xa level monitoring in patients with severe renal impairment, extreme body weight, or during pregnancy.
- Educate patients on proper injection technique and signs of bleeding.
Patient Guidelines
- Inject exactly as directed by your doctor.
- Do not rub the injection site after administration.
- Report any unusual bleeding or bruising immediately.
- Inform your doctor or dentist you are taking enoxaparin before any surgery or dental procedure.
Missed Dose Advice
If a dose is missed, administer it as soon as possible. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Enoxaparin is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Avoid activities that increase the risk of injury or bleeding.
- Use a soft toothbrush and an electric shaver to minimize bleeding risk.
