Overview of the medicine
Eramox 400 mg injection contains Amoxicillin, a penicillin-type antibiotic used to treat various bacterial infections. It works by stopping the growth of bacteria. It is commonly prescribed for respiratory tract infections, urinary tract infections, skin and soft tissue infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Typical dose for adults is 500 mg every 8 hours or 1000 mg every 12 hours, administered intravenously. Specific dosage for 400 mg injection would depend on the infection severity and local guidelines, often administered multiple times daily.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in elderly patients with reduced renal function.
Renal_impairment
Dosage reduction is required for patients with severe renal impairment (creatinine clearance <30 mL/min). For example, 500 mg every 12-24 hours.
How to Take
Eramox 400 mg injection is administered intravenously (IV) or intramuscularly (IM). For IV administration, it should be diluted in an appropriate intravenous fluid and infused slowly over 3-4 minutes or as an infusion over 30-60 minutes. IM administration should be deep into a large muscle.
Mechanism of Action
Amoxicillin is a bactericidal antibiotic. It acts by inhibiting bacterial cell wall synthesis. Specifically, it binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains. This leads to impaired cell wall formation, increased osmotic pressure, and ultimately bacterial cell lysis and death.

Onset
Rapid onset, typically within minutes after IV administration.
Excretion
Primarily excreted unchanged by the kidneys, through both glomerular filtration and tubular secretion. Approximately 60-70% of a dose is excreted in the urine within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed after oral administration; for injection, bioavailability is 100%. Peak plasma concentrations achieved quickly after IV administration.
Metabolism
Limited metabolism; about 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- •Hypersensitivity to penicillin antibiotics or any component of the formulation.
- •History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other beta-lactam antibiotic (e.g., cephalosporins, carbapenems).
Drug Interactions
Probenecid
Increases and prolongs Amoxicillin plasma levels by decreasing renal tubular secretion.
Methotrexate
Increases Methotrexate toxicity by reducing its renal clearance.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (rarely reported).
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time and increase the risk of bleeding.
Storage
Store the powder for injection below 25°C, protected from moisture and light. After reconstitution, the solution should be used immediately or stored as per manufacturer's instructions, typically for a short period (e.g., 6 hours at room temperature or 24 hours in a refrigerator).
Overdose
In case of overdose, symptoms may include gastrointestinal disturbances (nausea, vomiting, diarrhea), and in rare cases, renal dysfunction, crystalluria, or convulsions. Treatment is largely symptomatic and supportive. Hemodialysis can be used to remove Amoxicillin from the circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Amoxicillin is generally considered safe for use during pregnancy when clearly needed. It is excreted in human milk in small amounts, generally considered compatible with breastfeeding, but caution is advised.
