Eramox 400 mg Tablet contains Amoxicillin, a broad-spectrum penicillin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
For most infections, 400 mg every 8-12 hours or 500 mg every 8 hours, or 875 mg every 12 hours. For 400 mg tablet specifically, common regimens include 400 mg every 8 hours (for severe infections) or 400 mg every 12 hours (for mild to moderate infections).
Elderly
Similar to adult dosage, but renal function should be assessed, and dosage adjusted if necessary.
Renal_impairment
Dosage reduction is required. For CrCl 10-30 mL/min, 250-500 mg every 12 hours; for CrCl <10 mL/min, 250-500 mg every 24 hours. Max 500 mg every 12 hours.
How to Take
Take orally with or without food. For dispersible tablets, dissolve the tablet in a small amount of water (e.g., 10-20 mL) and drink immediately. Do not chew or crush non-dispersible tablets.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 30-60 minutes after oral administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Approximately 60-80% of an oral dose is excreted in urine within 6 hours.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function, prolonged in renal impairment.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, achieving peak plasma concentrations in about 1-2 hours. Bioavailability is approximately 70-90%.
Metabolism
Limited hepatic metabolism; about 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- •Known hypersensitivity to penicillin antibiotics.
- •Previous severe immediate hypersensitivity reaction (e.g., anaphylaxis) to another beta-lactam antibiotic (e.g., cephalosporins, carbapenems, monobactams).
Drug Interactions
Probenecid
Reduces renal tubular secretion of amoxicillin, resulting in increased and prolonged blood levels of amoxicillin.
Allopurinol
Increased incidence of rash in patients receiving both amoxicillin and allopurinol.
Methotrexate
Amoxicillin may reduce the renal excretion of methotrexate, increasing its toxicity.
Oral Contraceptives
May decrease the efficacy of oral contraceptives; advise additional non-hormonal contraception.
Warfarin and other anticoagulants
May prolong prothrombin time (INR). Increased monitoring of INR is recommended.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. In cases of massive overdose, renal failure and crystalluria may occur. Treatment is generally supportive and symptomatic. Gastric lavage may be considered soon after ingestion. Amoxicillin can be removed from circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Limited data in pregnant women indicate no increased risk of major birth defects. Amoxicillin is excreted in breast milk in small amounts; generally considered safe for nursing infants, but monitor for diarrhea, thrush, or rash.
