Overview of the medicine
Anidulafungin (Eraxis) is an echinocandin antifungal used to treat serious fungal infections such as invasive candidiasis, including candidemia, and esophageal candidiasis. It targets the fungal cell wall.
Uses & Indications
Dosage
Adults
For Invasive Candidiasis: 200 mg loading dose on Day 1, then 100 mg daily. For Esophageal Candidiasis: 50 mg daily. All doses by IV infusion.
Elderly
No dosage adjustment necessary.
Renal_impairment
No dosage adjustment necessary.
How to Take
Administer as a slow intravenous infusion over 1 hour. Do not administer as a bolus injection.
Mechanism of Action
Anidulafungin inhibits the synthesis of 1,3-β-D-glucan, an essential component of the fungal cell wall, leading to osmotic instability and cell lysis.
Pharmacokinetics
Onset
Rapid onset of action; antifungal effects observed within hours.
Excretion
Primarily eliminated via fecal excretion of intact drug and metabolites; less than 10% excreted renally.
Half life
Approximately 24-26 hours.
Absorption
Administered intravenously, resulting in 100% systemic bioavailability.
Metabolism
Undergoes slow chemical degradation at physiological pH and temperature; not extensively metabolized by hepatic cytochrome P450 enzymes.
Side Effects
Contraindications
- •Known hypersensitivity to anidulafungin or any component of the formulation
- •Known hypersensitivity to other echinocandins (e.g., caspofungin, micafungin)
Drug Interactions
Tacrolimus
No clinically significant interaction observed.
Cyclosporine
Co-administration does not require anidulafungin dose adjustment.
Voriconazole
No significant pharmacokinetic interaction; no dose adjustment needed.
Storage
Store unopened vials refrigerated at 2°C to 8°C (36°F to 46°F). Protect from light. Do not freeze.
Overdose
In clinical trials, no dose-limiting toxicity was observed up to 260 mg single dose. Management should be supportive. Anidulafungin is not dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution advised during breastfeeding as it is unknown if anidulafungin is excreted in human milk.
