Description
Overview of the medicine
Erythromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections, including respiratory tract infections, skin infections, and some sexually transmitted infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typical dose 250-500 mg every 6-12 hours, depending on infection severity. For children, follow pediatric guidelines.
Elderly
Similar to adult dosage; however, monitor for adverse effects due to potential age-related organ impairment.
Renal_impairment
No dose adjustment is generally needed for mild to moderate renal impairment. For severe impairment, caution is advised, and monitoring is recommended.
How to Take
Shake the suspension well before each use. Administer orally, preferably on an empty stomach for better absorption, or with food if gastrointestinal upset occurs. Use a calibrated measuring spoon or cup to ensure accurate dosing.
Mechanism of Action
Erythromycin inhibits bacterial protein synthesis by reversibly binding to the 50S ribosomal subunit of susceptible microorganisms, thereby blocking the translocation reaction.
Pharmacokinetics
Onset
Within 1 hour for oral administration.
Excretion
Mainly excreted in bile/feces; minor renal excretion.
Half life
Approximately 1.5 to 2 hours.
Absorption
Oral absorption is variable and incomplete; enhanced by fasting for base erythromycin. Ester forms (like ethylsuccinate) are better absorbed and less affected by food.
Metabolism
Primarily hepatic via CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to erythromycin or any other macrolide antibiotic.
- •Co-administration with terfenadine, astemizole, cisapride, pimozide, or colchicine (risk of serious cardiac arrhythmias or other severe adverse effects).
Drug Interactions
Digoxin
Increased digoxin levels, potentially leading to toxicity.
Warfarin
Increased anticoagulant effect, leading to increased bleeding risk.
Theophylline
Increased theophylline levels, leading to toxicity.
Carbamazepine, valproate
Increased anticonvulsant levels.
Cyclosporine, tacrolimus
Increased immunosuppressant levels and nephrotoxicity risk.
Statins (e.g., simvastatin, lovastatin)
Increased risk of myopathy/rhabdomyolysis.
Storage
Store dry powder below 30°C, protected from light and moisture. Keep out of reach of children. After reconstitution, store in a refrigerator (2-8°C) and do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal cramps, and diarrhea. Hepatic dysfunction and hearing loss may occur with very high doses. Management involves symptomatic and supportive care, including gastric lavage or induced emesis if ingestion is recent. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Erythromycin is generally considered safe for use during pregnancy, but should be used only if clearly needed. It is excreted in breast milk; caution should be exercised when administered to a nursing mother. Monitor for potential effects on the infant (e.g., gastrointestinal symptoms).
