Description
Overview of the medicine
Erymex 500 mg Tablet contains Erythromycin, a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin infections, sexually transmitted infections, and certain gastrointestinal infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typically 250 mg to 500 mg every 6 to 12 hours, or 500 mg twice daily for 7 to 14 days, depending on the severity and type of infection. Max dose usually 4 g/day.
Elderly
No specific dose adjustment for elderly patients with normal renal and hepatic function. Use adult dosage with caution.
Renal_impairment
No dose adjustment is usually necessary for mild to moderate renal impairment. For severe renal impairment (CrCl < 10 mL/min), maximum dose should not exceed 2 g/day.
How to Take
Take the tablet orally, usually with a glass of water. It can be taken with or without food, but taking it with food may reduce gastrointestinal upset. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Erythromycin binds reversibly to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis. This action is primarily bacteriostatic, but can be bactericidal at higher concentrations against highly susceptible organisms.

Onset
Antibacterial effect typically observed within hours of administration.
Excretion
Mainly excreted in bile and feces as active drug and metabolites; a small percentage is excreted in urine.
Half life
Approximately 1.5 to 2 hours, can be prolonged in hepatic impairment.
Absorption
Oral absorption of erythromycin base varies and is affected by food; enteric-coated formulations improve absorption. Peak plasma concentrations are achieved within 1-4 hours.
Metabolism
Primarily hepatic, via the cytochrome P450 3A4 (CYP3A4) isoenzyme system.
Side Effects
Contraindications
- •Hypersensitivity to erythromycin or any other macrolide antibiotics
- •Concomitant use with terfenadine, astemizole, cisapride, pimozide, ergotamine, or dihydroergotamine due to risk of serious cardiac arrhythmias
- •Pre-existing QT prolongation or uncorrected hypokalemia/hypomagnesemia
- •Severe hepatic impairment
Drug Interactions
Digoxin
Increased serum digoxin levels, leading to digitalis toxicity.
Warfarin
Potentiation of anticoagulant effects, requiring closer monitoring of INR.
QT prolonging drugs
Increased risk of QT prolongation and ventricular arrhythmias.
Cyclosporine, Tacrolimus
Increased plasma concentrations of immunosuppressants, leading to increased toxicity.
Statins (e.g., simvastatin, atorvastatin)
Increased risk of myopathy/rhabdomyolysis due to inhibition of CYP3A4-mediated metabolism.
Ergot alkaloids (ergotamine, dihydroergotamine)
Acute ergot toxicity characterized by vasospasm leading to ischemia of extremities and other tissues.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, abdominal pain, diarrhea, and hearing loss (reversible). Gastric lavage and supportive care are recommended. Monitor cardiac function due to risk of QT prolongation.
Pregnancy & Lactation
Pregnancy Category B. Erythromycin is generally considered safe for use during pregnancy when clearly needed. Small amounts are excreted in breast milk; generally considered compatible with breastfeeding, but monitor the infant for gastrointestinal upset.
