Overview of the medicine
Esoprol MUPS is an anti-ulcerant drug containing Esomeprazole, a proton pump inhibitor, used to reduce stomach acid production and treat acid-related conditions like GERD, peptic ulcers, and Zollinger-Ellison syndrome. The MUPS formulation allows for improved absorption and therapeutic effects.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 40 mg once daily with antibiotics for 10 days.
Elderly
No dosage adjustment generally required, but caution is advised in very frail patients.

How to Take
Take orally, at least one hour before a meal. Capsules should be swallowed whole. If swallowing is difficult, the capsule can be opened and the pellets mixed with a small amount of water or fruit juice and swallowed immediately. Do not chew or crush the pellets.
Mechanism of Action
Esomeprazole inhibits the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion.

Primarily via urine (about 80%) and feces (about 20%) as metabolites.

Absorption
Rapidly absorbed after oral administration; bioavailability is about 64% after a single 40 mg dose.

Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with Nelfinavir.

Digoxin
Increased absorption of digoxin.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased levels and prolonged exposure to methotrexate.
Atazanavir, Nelfinavir
Decreased plasma levels of antiretrovirals.
Storage
Store in a cool, dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience. Symptoms include drowsiness, tachycardia, headache, blurred vision, abdominal pain, diarrhea. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and potential benefits outweigh risks. Caution advised during lactation as Esomeprazole is likely excreted in human milk.
